Issue:
Based on interview and record review, the facility failed to prevent unauthorized access and disclosure of a patient's (Patient 5) protected, confidential information, when Patient 5's license and insurance information was given to a person not authorized to receive that information. This incident allowed for potential unlawful and/or unauthorized use of the identification information.Findings: During an interview on 11/01/13 at 10 a.m., the Compliance Officer confirmed that the patient's information had been given to the incorrect patient. She stated that Patient 5's license and insurance card were given to another patient when the other patient was discharged from the Emergency Department. The Compliance Officer confirmed the information which was released. The Information released included the patient's name, license number, address, identifying information, date of birth, and name and identification number of the insurance coverage for Patient 5.The staff person releasing the information failed to verify the identity of the patient prior to giving her the documents which belonged to Patient 5.
