Issue:

An electrocardiogram (ECG) rhythm strip with notes on the back was found on a sidewalk outside of the VA. It had 1 patient's information on it including the full name, full SSN, date of birth, and protected health information (PHI). It also 5 patients' information including the last name, age, last 4 digits of the SSN and PHI. There was a total of 6 patients involved. It was outside of VA control for less than 24 hours. Update: 03/14/12:Veteran A will be sent a letter offering credit protection services and Five (5) Veterans will be sent a notification letter.

Outcome:

Re-educated house staff on proper note taking and safeguarding PHI.
