Issue:

Two Veterans reported to facility Privacy Officer that information they had received in mail yesterday was mixed up; Veteran A received Veteran B information in his envelope; Veteran B not sure but did not receive entire packet of medical record information requested and is still awaiting delivery of additional requested information. Veteran A directly contacted Veteran B and both came in to Privacy Office to report breach of privacy and concerns related to their requests for release of information processed over last 3 workdays and received at home mailing addresses. Veteran A provided/returned entire packet to Privacy Officer with copy of envelope and correct information was verified and then provided to each Veteran. Both thanked for reporting the mix-up and process for reporting of privacy breach explained to both. Apologies offered to each and thanks for returning incorrect information and reporting this privacy issue. Issue details provided to responsible Manager for Release of Information Section for fact-finding and corrective action.

Outcome:

10/22/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services. It was confirmed that Veteran A's information was not sent to Veteran B.
