Issue:
A geriatric handoff report containing the protected health information (PHI) of two patients was discovered on the floor of a female rest room. The report was retrieved and brought to the attention of the Privacy Officer (PO) by a female employee. Update: 03/18/13:The restroom is open to the public and it is not known how long the report was left in there. Two patients will be sent a HIPAA notification letter.
