Issue:
On 06/29/12, VA Employee A from occupational health placed a letter containing patient lab results, full name, and full SSN on the clinic door inbox. Employee A placed the letter in the box so VA Employee B could mail the letter to the Veteran. The employee followed up to see if the records were retrieved and mailed by Employee B. The records werent retrieved. The letter containing the Veteran's Protected Health Information (PHI) and Personally Identifiable Information (PII) was taken out of the box by an unknown individual. Update: 07/02/12:The Veteran will be sent a letter offering credit protection services due to full name and full SSN being exposed.
Outcome:
Privacy Officer provided education and training to the individual responsible for the violation.
