Issue:

An employee left a message on an answering machine. In the message, the employee never asked the patient to call back to schedule an appointment. She stated that she had reviewed his record and saw that he had an appointment scheduled on 12/12/2012 for PTSD and that she would schedule the appointment with her clinic after the PTSD appointment. The patient is extremely upset that protected health information would be left on an answering machine for anyone else in the home to hear. Update: 12/06/12:One Veteran will be sent a letter of notification.

Outcome:

Education was provided to the employee, as well as a reminder message being sent to all staff regarding leaving messages.
