Issue:
Veteran A (name unknown) was mistakenly handed Veteran B's lab results by his Provider. Veteran A returned the lab report to the nearest CBOC and gave a brief explanation of what transpired. Due to task saturation and under manning at the time, the lone clerk at the check-in desk was unable to collect Veteran A's name nor any details surrounding the mis-handling of Veteran B's lab results. The lab report was dated 13 Sept. 2012. Update: 10/09/12:Veteran B will be sent a letter offering credit protection services due to full name and full SSN being exposed.
Outcome:
The Provider was counseled and re-educated by the Chief Medical Officer.
