Veteran A received Veteran B's Fee Based Care denial letter. Veteran A called the VAMC to report the inappropriate disclosure and that Veteran A had already shredded the material. Update: 06/29/12:Veteran B will be sent a letter of notification.
Outcome:
The supervisor of the employee who mismailed the letter will re-educate the employee on the need to protect Veteran PHI and PII.
