Issue:
COTR requested SPAR after experiencing identity theft issues, unrelated to employment. After SPAR was provided, COTR initiated privacy complaint to PO. Allegation of inappropriate access to his CPRS chart by a subcontractor. HealthNet's subcontracted provider accessed COTR's chart in CPRS per SPAR on March 14, 2011 x 2 at 17:34 and 17:36. Appropriate notification and investigation to ensue. Update: 07/26/11:The COTR will be sent a letter offering credit protection services, due to full name and full SSN being accessed inappropriately.
Outcome:
Contractor provided additional privacy education and information to the provider. Credit monitoring offer to COTR.
