Issue:
Medications for Patient A were mailed to Patient B. The wife of Patient B called to inform facility that they had received medications for another Veteran, she stated that they would return the medications. The information disclosed was the name, address, scrambled date of birth, name of physician and medication directions. Update: 06/07/11:Patient A will be sent a notification letter, due to name and PHI being exposed.
Outcome:
Pharmacy Supervisor to in-service staff to be more diligent in checking medications before shipping to prevent unauthorized disclosures.
