Issue:

The Thoracic Oncology Conference was held on 10/02/12 in a VA conference room. Someone left a list of the patients that were discussed during the conference in the conference room. It contained two (2) Veterans' information. It included name, age, last four of the SSN, dates, diagnosis, clinic review, and pending tests. Update: 10/09/12:The list was left in a conference room which was accessible to the general public. The room was used by a group of Veterans after the conference and the list was found by the Veterans. Two Veterans will be sent a letter of notification.

Outcome:

Chief of Medicine services educated attendees on the importance of protecting Veterans privacy. He will ensure all forms are collected and/or shredded as appropriate after each meeting.
