Issue:

Today, 4/24/2014, Clerk reports a privacy incident that a Veteran provided to her for this purpose. Veteran A requested his medical records from the Release of Information window on-campus. He went to his outpatient appointment. While there, he realized that all 37 pages of medical records were for Veteran B. He turned over records to the outpatient Clerk to report to PO. He is expecting the remainder of his records via mail because he was told old records were not available today. Review of the records shows root cause of records provided. Both Veteran A and B have the same last 4 of SSN with same initial of last name; however, names are different. Additional notification and investigation, pending.

Outcome:

04/24/14: The Incident Resolution Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
