Veteran A received a FOBT kit from a contracted Community Based Outpatient Clinic (CBOC) employee that had the full name, full Social Security Number, and birth date of Veteran B. Veteran A returned the kit to the contracted CBOC.
Outcome:
10/20/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
