Issue:
Pharmacy notified the Privacy Officer (PO) that Veteran A received his medication and also received Veteran B's medication in the same bag. Veteran A at first did not notice it. Veteran A was on insulin when he was in the hospital and at that time noticed it was a different name and still took the insulin. When he went to the nursing clinic days later the nurse noticed it and asked the Veteran about it. Veteran A stated that he took the insulin. The nurse notified the Primary Care Provider for Veteran A to examine him. Update: 06/28/12:Veteran B will receive a letter of notification.
Outcome:
Education was conducted in addition to continued quality reviews.
