Issue:

Veteran A contacted the Pharmacy Service staff on 06/19/12 regarding not receiving his medication (Oxycodone). The Pharmacy Service staff member contacted the UPS representative who advised that the Pharmacy label was mislabeled. The medication was delivered to Veteran B. The Pharmacy staff contacted Veteran B who advised that he discarded the pills since he noticed another Veterans name on the bottle. The Pharmacy Service staff member advised him to contact the Pharmacy immediately if he receives the wrong medication in the future. Veteran A was provided a replacement medication on 06/19/12.Package included medication and instruction sheet with Veterans A full name and address and information on medication (oxycodone). Update: 06/27/12:Veteran A will be sent a notification letter due to PHI being exposed.

Outcome:

The VA Police Chief has been notified for further investigation.
