Issue:
Patient A notified the VA that he had received a medication that belonged to Patient B. Patient A was sent a return envelope and has returned the medication for Patient B. Patient A returned the medication to the VA. This medication will be destroyed since it has already left our possession. The information included on the medication was the Patients full name, RX#, Medication, and Diagnosis. Update: 04/20/12:Patient B will receive a letter of notification.
