Issue:

Prescription and medication lists and supplies for two patients were found in a dumpster located outside a local store.. Documents and materials are still being recovered and investigated further by VA Staff. The documents contained the patients' name, full SSN and medications. Update: 04/21/11:The three (3) Veterans whose full names and SSNs were exposed will be sent letters offering credit protection services. One (1) Veteran whose full name and PHI was exposed will be sent a letter of notification.

Outcome:

The following is what is currently being provided for corrective action: 1. All HBPC nursing staff are being assigned the privacy and IS security training through LMS2. A locked cabinet will be provided to the HBPC nurses office in the CBOC3. Shred bin was provided to the HBPC nurse today to bring to their CBOC office
