Issue:
A physician had a list of patients that he was working on in his locked office. The physician locked the office and returned to find the list missing. The office is shared with several other providers. The full SSN, full name and Protected Health Information (PHI) was contained on the list. Approximately 100 patients are involved. The facility is still attempting to determine all the patients that were on the list. The facility has conducted an extensive search and were unable to recover the PHI. Update: 08/30/11:The 100 patients will receive a letter offering credit protection services .09/01/11:Further investigation reveals that the total is 132 patients on the list. The 132 patients will receive a letter offering credit protection services.
Outcome:
PO have taken corrective action - removing logbook, remedial privacy training.
