Issue:
A Veteran received two letters addressed to him, one contained his filled out form requesting more information. The second contained another Veteran's filled out form requesting more information. The Veteran returned information to Information Security Officer (ISO) office. Update: 05/12/11:The form was a VA Form 10-10EZR and they were partially filled out by the Veterans. The office was sending them back to the Veterans for additional financial information. The forms contained the Veterans name, gross annual income, net income, and other income amounts along with name, SSN, and date of birth. One Veteran will receive a letter offering credit protection services.
