Issue:

The report states that on 06/08/11, Veteran A received Veteran Bs prescription. The Veterans have the same last name. The report states that the medication was mailed from Providence. Actions, Progress and Resolution Date: The information disclosed contained Veteran B's full name, home address and contact number. The Privacy Officer (PO) checked the records of both Veterans and determined that both live in Warwick. The address printed on Veteran Bs prescription sheet was correct. It is unclear if this was a mistake made by mail delivery or Providence VAMC. Update: 07/25/11:Veteran B will be sent a notification letter, since his name and medication type were disclosed.

Outcome:

Notification letter sent. Employee required to retake privacy training.
