Issue:

Social Worker (SW)was en route to Veterans Court Docket (walking to vehicle at the MEDVAMC) while carrying a folder with the future appointment lists for the ~56 veterans enrolled in the court. SW regularly distributes the appointment lists to the Veterans to keep them updated on changes in their local VA appointments. Upon walking on the sidewalk to her vehicle, Social Worker tripped, fell and folder was knocked from her arms. Several of the lists fell out of the manila folder (those towards the end of the alphabet W-Z) no others left the folder. SW quickly picked up the loose appointment lists placed them back in the folder and carefully scanned area to see if any other lists went missing, and then proceeded to vehicle. Upon distributing the lists at the docket to the individual Veterans, SW noticed that she did not have the appointment list for one Veteran SW completed search of area where she fell immediately upon return from court and her office to ensure that she possibly left his list behind but could not locate list. Appointment list from Ad Hoc Summary in DHCP contains full SS# and Date of Birth. SW is unsure whether she may have just not printed the missing appointment list but to be careful reported incident.

Outcome:

01/10/14: The one Veteran whose information was potentially lost will be offered credit protection services.
