Issue:

A Veterans wife, who is a Palmetto Health Richland RN, called the Dorn VA Medical Center Release of Information (ROI) Office to inform them that her husband received a copy of his Radiology report, and a two page list of names and social security numbers of seventeen other Veterans. The list was inadvertently mailed to the Veteran by the ROI Office. A formal letter has been prepared for the Medical Center Directors signature, requesting that the Veteran return the two page document to the Dorn VAMC. The information disclosed was seventeen Veteran's full names and full social security numbers. The error apparently occurred on 02/17/12. Update: 03/15/12:A letter offering credit protection services will be sent to 17 Veterans due to full name and SSN being disclosed.

Outcome:

HR is working on the disciplinary action regarding the Release of Information Office employee who inadvertently mailed a spreadsheet with 17 Veterans names & ssns; She hopes to have the letter completed by Thursday for review and concurrence by the Chief of HR.
