Issue:

The Providence VA Medical Center (PVAMC) sends Dermatology patients to a local hospital for surgery appointments, that hospital sends the results back to the VA Dermatology department. Yesterday, a resident physician was instructed to bring the packet of patient information to the PVAMC. Since it was after hours, and the resident was scheduled to be at the PVAMC first thing this morning, she took the packet home overnight. It was stolen along with a backpack and other items from her home. The resident called Providence police and filed a report, which she will provide to the PVAMC Privacy Officer. She notified the civilian Dermatology's department, who notified the PVAMC immediately. Update: 10/29/12:Seven (7)) Patients information was stolen, therefore 7 Patients will receive a letter offering credit protection services.

Outcome:

Derm changed the way they are transporting the MOHS packets, this will provide for better security.
