Issue:
A VA social worker visited Veteran A at his residence today. Veteran A told the social worker he was confused about mail he received from VA. The social worker realized Veteran B's appointment letter was also included in the envelope with Veteran A's appointment letter. The social worker regained custody of the appointment letter and provided it to the Privacy Office.
Outcome:
05/01/14: The Incident Resolution Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
