Issue:
A Pharmacy technician gave the prescription bag for Patient B to Patient A. Patient A discovered the error after arriving home. Patient A did not open the bag, but observed the back page of the prescription label, which identified the bag's contents, and that's when she realized it wasn't hers. Patient A returned the bag to the Pharmacy the same day. The information at risk included patient B's name and type of medication. Update: 02/21/12:Patient B will be sent a notification letter due to PHI being exposed.
Outcome:
Employee was administered a verbal counseling and given a copy of the pharmacy services policy to review.
