Issue:

A clerk gave a pre-med record note to the wrong patient when he was checked in. When the nurse called him back, he said he was not sure why it has eye drops listed on my medication list as he does not use eye drops. The nurse looked at the med-rec note and determined it was on the wrong patient and took it from him.

Outcome:

04/07/14: The Incident Resolution Team has determined that there was a policy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise.
