Issue:
The Privacy Officer (PO) conducted an audit on requests that were closed in Document Storage Systems (DSS) Release of Information (ROI) as other-non disclosure to ensure they were processed in accordance with VHA Handbook 1605.1. A total of 70 requests were selected for the review and 25 out 70 requests could not be located in DocSearch. ROI conducted a search of their physical area and were able to locate and produce 9 additional requests. As of today, 16 requests are unaccounted for. Requests can include full names, full Social Security Numbers, dates of birth and medical conditions. The PO is unable to identify the exact information at risk.
Outcome:
05/01/15: The Incident Resolution Service Team has determined that 11 Veterans will be sent letters offering credit protection services.
