THE WRITTEN STATEMENT CONCERNING CONFLICT OF INTEREST IS CIRCULATED ANNUALLY TO ALL BOARD MEMBERS AND EMPLOYEES WHO ARE TO READ AND REVIEW THE DOCUMENT IN ITS ENTIRETY. UPON REVIEW, THEY ARE TO SIGN A STATEMENT EITHER GIVING ASSURANCE THAT THERE ARE NO CONFLICTS OF INTEREST OR LIST ANY POSSIBLE CONFLICTS OF INTEREST ON THE SPACE BELOW. IN THE EVENT OF ANY POSSIBLE CONFLICTS, THE BOARD REVIEWS THE STATEMENT AND MAKES A DETERMINATION TO EITHER REPLACE THE BOARDMEMBER OR FOR THEM TO IMMEDIATELY TERMINATE ANY TYPE OF TRANSACTION WHICH IS NOT APPROPRIATE. |
