FORM 990, PART VI, SECTION B, LINE 12C THE GOVERNING BOARD APPROVED A CONFLICT OF INTEREST POLICY DOCUMENT. KEY STAFF MEMBERS AND VOLUNTEER BOARD MEMBERS SIGN A CONFLICT OF INTEREST POLICY WHICH IS RETURNED TO THE BUSINESS OFFICE AND FILED IN THE SAFE. EACH PERSON RECEIVED A COPY OF THE CONFLICT OF INTEREST POLICY. WE EDUCATE ALL STAFF MEMBERS AND VOLUNTEERS ABOUT THE POLICY WITH THE EXPECTATION THAT ALL WILL COMPLY WITH THE POLICY. THERE IS AN UNDERSTANDING THAT THE DUTY TO DISCLOSE IS CONTINUOUS. FAILURE TO TIMELY AND ADEQUATELY RESPOND TO THE QUESTIONNAIRE OR TO MAKE ANY SUBSEQUENT DISCLOSURE IS GROUNDS FOR BOARD ACTION.
FORM 990, PART VI, SECTION B, LINE 15 THE SALARY FOR THE PRESIDENT/CEO IS SUBJECT TO THE FOLLOWING: THE EXECUTIVE COMMITTEE DETERMINES THE PRESIDENT/CEO SALARY. THE YMCA OF THE USA SUPPLIES OUR EXECUTIVE COMMITTEE A COMPARATIVE RANGE OF SALARIES FROM SIMILAR BUDGET SIZED YMCAS ACROSS THE COUNTRY. IN ADDITION, EXECUTIVE SALARIES FROM A KINDRED GROUP OF YMCAS ARE FACTORED IN. THESE ARE USED TO SET THE SALARY AFTER BENCHMARKING AGAINST PERFORMANCE OBJECTIVES. THE RESULTS OF THESE OBJECTIVES ARE THEN MEASURED AND BENCHMARKED AGAINST A MULTI-FACETED ASSESSMENT RATE ON KEY FUNCTIONAL AREAS. THE ORGANIZATION UTILIZES THE HAY POINTS SYSTEM FOR BOTH NON-EXEMPT AND EXEMPT POSITIONS. THE HAY POINTS SYSTEM IS A POINTS BASED EVALUATION TOOL TO DETERMINE THE POINT VALUE OF A POSITION. THE HAY POINTS SYSTEM FOR NON-EXEMPT POSITIONS CONSISTS OF TEN FACTORS UPON WHICH POSITIONS ARE ASSIGNED POINTS BASED ON THE LEVEL THAT BEST FITS THAT POSITION. TOTAL POINTS ARE CONSIDERED WHEN DETERMINING WHICH GRADE LEVEL THE HOURLY WAGE WILL BE ASSIGNED. THE ORGANIZATION DOCUMENTS NON-EXEMPT GRADE LEVELS AND COMMUNICATES THEM TO ALL SUPERVISORY STAFF. THE MINIMUM OF THE GRADE LEVEL CORRESPONDS TO THE FEDERAL AND WISCONSIN STATE MINIMUM WAGE, WHATEVER IS GREATER. THE HAYS POINTS SYSTEM FOR EXEMPT POSITIONS CONSISTS OF THREE BASIC FACTORS UPON WHICH POSITIONS ARE ASSIGNED POINTS AND LEVELS, BASED ON THE RESPONSIBILITIES AND SCOPE OF THAT POSITION. TOTAL POINTS ARE RECORDED AND ADDED TO A BASE SALARY FORMULA. WE UTILIZE THE ANNUAL RECOMMENDATIONS THAT THE YMCA OF THE USA PROVIDES FOR US REGARDING MARKET DATA AND RECOMMENDED POSITION POINTS AND BASE SALARY. THE COMPENSATION LEVELS OF EXEMPT AND NON-EXEMPT POSITIONS ARE CONTINUALLY REVIEWED BY THE HUMAN RESOURCES DEPARTMENT, CABINET COMMITTEES AND EXECUTIVE DIRECTOR TO ENSURE WE REMAIN COMPETITIVE IN THE MARKET.
FORM 990, PART VI, SECTION C, LINE 19 THE ORGANIZATION MAKES ITS GOVERNING DOCUMENTS, CONFLICT OF INTEREST POLICY AND FINANCIAL STATEMENTS AVAILABLE TO THE PUBLIC UPON REQUEST. THE TAX RETURN IS AVAILABLE ON GUIDE STAR AND IT CONTAINS THE TAX INFORMATION, WHICH INCLUDES THE FINANCIALS.
