PNHS PROVIDES CARE AND SUPPORT TO MINNEAPOLIS AND WEST METRO COMMUNITIES THROUGH AN ACUTE CARE HOSPITAL LICENSED FOR 426 BEDS, 593 FULL-TIME-EQUIVALENT PHYSICIANS PRACTICING 45 SPECIALTIES AT 20 PRIMARY CARE CLINICS, 2 AMBULATORY SURGERY CENTERS, 11 RETAIL PHARMACIES AND 8 HEALTHCARE PRODUCT RETAIL STORES. AS A RESULT OF THE COMBINATION OF HEALTHPARTNERS AND PNHS, A NEW LOGO WAS ESTABLISHED AS A VISUAL SYMBOL OF THIS RELATIONSHIP. HPI HAS AN OUTSTANDING TRADITION OF CONSUMER GOVERNANCE. THE PROGRAM REACHES OUT TO ADOLESCENTS ENROLLED IN HPCARE BY SENDING BIRTHDAY REMINDERS TO SCHEDULE A CHECKUP EXAM. IF THE PROVIDER COMPLETES THE EXAM AND SIGNS THE REMINDER POSTCARD, THE CHILD RECEIVES A $25 TARGET GIFT CARD. THE 2 YEAR IMMUNIZATION INCENTIVE PROGRAM WAS DEVELOPED TO INCENT FAMILIES TO HAVE ALL OF THEIR CHILD'S IMMUNIZATIONS UP TO DATE BY AGE 2. A $25 GIFT CARD IS SENT TO THE FAMILY WHEN THE IMMUNIZATIONS ARE COMPLETE. - THE BLOOD LEAD INCENTIVE PROGRAM. THE BLOOD LEAD INCENTIVE PROGRAM WAS ESTABLISHED TO PROVIDE OUTREACH AND BLOOD LEAD TESTING TO FAMILIES WITH BABIES AND YOUNG CHILDREN ENROLLED IN HPCARE. THE BLOOD LEAD INCENTIVE PROGRAM PROVIDES A $25 TARGET GIFT CARD TO HPCARE FAMILIES WHO RECEIVE A BLOOD LEAD TEST. HEALTHPARTNERS DISTRIBUTED INCENTIVE GIFT CARDS TO ENCOURAGE MEMBERS TO RECEIVE BLOOD LEAD TESTS. HEALTHPARTNERS PARTICIPATED IN EFFORTS TO PROVIDE EDUCATION AND OUTREACH AROUND BLOOD LEAD TESTING AND CHILD AND TEEN CHECKUPS INITIATIVES THROUGH MINNESOTA. MEDICAL EDUCATION HEALTHPARTNERS INSTITUTE FOR EDUCATION AND RESEARCH ("THE INSTITUTE") IS A MINNESOTA NONPROFIT CORPORATION RECOGNIZED AS EXEMPT FROM FEDERAL INCOME TAX UNDER INTERNAL REVENUE CODE (IRC) SECTION 501(C)(3). THE INSTITUTE'S MISSION IS TO IMPROVE HEALTH AND WELL-BEING IN PARTNERSHIP WITH OUR MEMBERS, PATIENTS AND COMMUNITY. THE INSTITUTE PROVIDES NATIONALLY ACCREDITED GRADUATE MEDICAL EDUCATION, CONTINUING MEDICAL EDUCATION, CLINICAL SIMULATION, AND OTHER HEALTH PROFESSIONAL EDUCATION. AGAIN, HEALTHPARTNERS WAS A GOLD SPONSOR OF MINNESOTA'S NAMI WALK, AN EVENT TO RAISE AWARENESS AND REDUCE THE STIGMA ASSOCIATED WITH MENTAL ILLNESS. AS AN INTERNAL COMMUNITY EFFORT, HEALTHPARTNERS EMPLOYEES CONTRIBUTED, IN ADDITION TO OUR SPONSORSHIP, $21,000 IN DONATIONS. - MAKE IT OK CAMPAIGN. HEALTHPARTNERS TEAMED WITH THE NATIONAL ALLIANCE ON MENTAL ILLNESS (NAMI) MINNESOTA AND PRESTON KELLY, A CREATIVE ADVERTISING AGENCY TO CREATE A NEW CAMPAIGN. "MAKE IT OK" IS A CAMPAIGN TO REDUCE THE STIGMA OF MENTAL ILLNESSES. THE ORGANIZATIONS INVOLVED HAVE COMMITMENT TO CHANGE THE MISPERCEPTIONS OF MENTAL ILLNESSES BY ENCOURAGING OPEN CONVERSATIONS AND EDUCATION ON THE TOPIC. THAT REPRESENTS A 42% INCREASE IN SUBMITTED APPLICATIONS OVER 2012. MN COMMUNITY MEASUREMENT HEALTHPARTNERS HELPED TO ESTABLISH MN COMMUNITY MEASUREMENT (MNCM) WITH OTHER MINNESOTA HEALTH PLANS AND THE MINNESOTA MEDICAL ASSOCIATION, AND PROVIDES FINANCIAL SUPPORT TO MNCM ON AN ANNUAL BASIS. HEALTHPARTNERS CONTRIBUTED $202,069 IN 2013. MEASURES INCLUDE HEALTH OUTCOMES, PATIENT EXPERIENCE, AND COST OF CARE. PRIMARY CARE AND SELECT SPECIALTIES ARE INCLUDED IN THE MEASUREMENTS. IN 2013, YUMPOWER SCHOOL CHALLENGE 2.0 WENT INTO EFFECT. WE COLLABORATED WITH RADIO DISNEY TO KICK OFF THE SCHOOL CHALLENGE 2.0 WITH A 45-MINUTE, HIGH-ENERGY, FUN SCHOOL ASSEMBLY. THE SPECIAL EVENT MADE EATING FRUITS AND VEGETABLES COOL, AND ENCOURAGED HIGH PARTICIPATION. OVER 75 PERCENT OF STAFF SURVEY RESPONDENTS RATED THE KICK-OFF ASSEMBLY AS EXCELLENT -AND THE DISNEY MUSIC DANCE DVD WAS ONE OF THE MOST HIGHLY RATED PORTIONS OF THE PROGRAM. DISNEY APPEARS TO BE "MAGIC" IN GENERATING EXCITEMENT FOR FRUITS, VEGGIES, AND THE SCHOOL CHALLENGE AS A WHOLE. DURING THE 2013 YUMPOWER SCHOOL CHALLENGE 2.0, STUDENTS RECEIVED A FOUR-WEEK SUPPLY OF FRUIT AND VEGETABLE TRACKERS WITH COLORFUL STICKERS AND INFORMATION. STUDENTS USED THE STICKERS TO TRACK HOW MANY FRUITS AND VEGETABLES THEY ATE EACH DAY, INCLUDING AT-HOME TRACKER PADS FOR OVER THE WEEKENDS. EACH WEEK, STUDENTS WOULD TURN IN THEIR COMPLETED TRACKER FOR A SMALL PRIZE (A PENCIL, RULER, NOTEPAD, DRY ERASE BOARD). SCHOOL LEADERSHIP, TEACHERS, SCHOOL NURSES, FOOD SERVICE EMPLOYEES AND SCHOOL VOLUNTEERS WERE ESSENTIAL IN MAKING THIS PROGRAM A SUCCESS. THIS PROGRAM REQUIRES CONSISTENT COMMUNICATIONS AND PROMOTION TO STUDENTS IN ORDER TO HAVE THE GREATEST IMPACT. TAILORING THE PROGRAM TO EASILY FIT INTO TEACHERS' CURRICULUM IS ALSO PARAMOUNT TO ITS SUCCESS. THIS THREE-YEAR INITIATIVE FOCUSES ON SPECIFIC ETHNIC COMMUNITIES IN MINNEAPOLIS - LATINO IN THE PHILLIPS NEIGHBORHOOD AND SOMALI IN RIVERSIDE - AND THE WHITE BEAR LAKE COMMUNITY AS A WHOLE. THE WEEKLY PROGRAM IS OPERATED IN COLLABORATION WITH PILLSBURY UNITED COMMUNITIES - WAITE HOUSE. WEEKLY NUTRITION EDUCATION AND FITNESS CLASSES WERE OFFERED FOR ADULTS AND YOUTH, WITH CHILDCARE AVAILABLE. WEEKLY ATTENDANCE AVERAGED 50 - 65 PARTICIPANTS. IN ADDITION, A GROUP OF PUBLIC HEALTH, COMMUNITY AND HEALTH ORGANIZATIONS MET REGULARLY TO COLLABORATE ON SHARED MESSAGES AND HEALTH IMPROVEMENT STRATEGIES. CEDAR-RIVERSIDE HEALTHPARTNERS AND CHILDREN'S HOSPITALS AND CLINICS ARE EXPLORING OPPORTUNITIES FOR PARTNERSHIP WITH ORGANIZATIONS IN THE CEDAR-RIVERSIDE NEIGHBORHOOD. IN 2013, PROVIDERS IN THE OB/GYN AND PEDIATRIC DEPARTMENTS BEGAN INVESTIGATING CLINIC AND COMMUNITY BASED OPPORTUNITIES TO INTEGRATE THEIR MESSAGE AND PRACTICE ON THE TOPIC OF EARLY INFANT FEEDING PRACTICES. HEALTHPARTNERS AND CHILDREN'S HOSPITALS AND CLINICS HAVE BUILT RELATIONSHIPS WITH LOCAL ORGANIZATIONS WORKING IN COMMUNITY HEALTH IMPROVEMENT AND WILL BE EXPLORING FORMAL PARTNERSHIPS IN 2014.
