We respectfully request any and all penalties for filing Form 990 late be abated. Several mitigating factors took place that resulted in this late filing. The Director of the Institute suffered a heart attack and underwent bypass surgery as well as the Financial Officer incurring significant health issues at the same time. The result of this caused the audited financial statements to be later than usual and also delaying the filing of the Form 990. There was turnover in the accounting staff during this time period as well, leaving many duties to be covered. An attempt to file a valid extension was made; however, it was rejected from e-filing as the due date had already passed. The Institute has always filed a timely Form 990 and these circumstances were beyond their control. Please consider all of these items as reasonable cause for the late filing.
