C.OPEN HAND'S PANTRY PROGRAM PROVIDES FOR A WEEKLY SUPPLY OF BOTH NONPERISHABLE GROCERIES, FRESH FRUIT AND VEGETABLES TO LOW-INCOME, MEDICALLY-ELIGIBLE CLIENTS WHO ARE EMPOWERED TO PREPARE THEIR OWN HEALTHY MEALS WITH NUTRITION GUIDANCE PROVIDED BY OPEN HAND REGISTERED DIETITIANS. THROUGH THE MARKET BASKET PILOT PROGRAM, OPEN HAND PROVIDED PANTRY MEALS TO 47 NEW CLIENTS IN NEED OF SUPPLEMENTAL FOOD AND NUTRITION ASSISTANCE WITH A NUTRIENT TARGETED MARKET BASKET THAT ASSISTS CLIENTS WITH DEVELOPING COOKING SKILLS. D.IN ADDITION, OPEN HAND PROVIDED APPROXIMATELY 3,663 EMERGENCY NUTRITION SUPPLEMENTS (ENS), IN THE FORM OF MEAL REPLACEMENT SNACKS AND NUTRITION SUPPLEMENTS, TO LOW-INCOME, MEDICALLY-ELIGIBLE MEN, WOMEN AND CHILDREN, OFTEN DURING EXTENDED WAITS FOR THEIR HEALTH CARE APPOINTMENTS. PATIENTS WHO HAVE HAD DENTAL PROCEDURES MAY ALSO RECEIVE MECHANICAL SOFT MEALS PROVIDED THROUGH THIS PROGRAM. E.CENTRAL TO OPEN HAND'S MISSION OF COMPREHENSIVE NUTRITION CARETM, EVERY CLIENT IS ASSESSED AT INTAKE AND OFFERED CONSISTENT NUTRITION EDUCATION BY OPEN HAND REGISTERED DIETITIANS (RDS), WHICH INCLUDES PRINTED NUTRITION EDUCATION MATERIALS AND, DEPENDING UPON THEIR ASSESSMENT, INDIVIDUAL OR GROUP NUTRITION COACHING SESSIONS. THESE SESSIONS HAVE A COMMUNITY OR PUBLIC HEALTH FOCUS, AND INCLUDE TOPICS SUCH AS PORTION CONTROL, HEALTHY EATING, HANDS-ON FOOD PREPARATION, PHYSICAL ACTIVITY, GOAL-SETTING, AND DISEASE SELF-MANAGEMENT. FOLLOW-UP REINFORCEMENT IS PROVIDED BY RDS, AND MAY INCLUDE REVIEW SESSIONS, GROUP TRAINING, REFERRAL(S) TO ADDITIONAL SUPPORT SERVICES, AND/OR PHONE CONSULTATIONS. F.OPEN HAND PROVIDED MEDICAL NUTRITIONAL THERAPY (MNT) TO 317 CLIENTS IN 2013. MNT IS PRESCRIBED BY A CLINICIAN, TARGETS A SPECIFIC MEDICAL CONDITION, AND INCLUDES A NUTRITION DIAGNOSIS, THERAPY AND COUNSELING SERVICES - ALL PERFORMED BY OPEN HAND REGISTERED DIETITIANS (RDS). DESIGNED BY THE AMERICAN ACADEMY OF NUTRITION AND DIETETICS, MNT INCLUDES A SPECIFIC PLAN OF ASSESSMENT, NUTRITION DIAGNOSES, INTERVENTION, EVALUATION/MONITORING, DOCUMENTATION, AND COMMUNICATION WITH CLIENT'S HEALTH CARE PROVIDER. THIS SERVICE IS AVAILABLE TO MEN, WOMEN AND CHILDREN WHO HAVE A NUTRITION-SENSITIVE CHRONIC DISEASE AND WHO ARE COMMITTED TO IMPROVING THEIR HEALTH THROUGH BETTER FOOD CHOICES AND MODIFIED BEHAVIORS. |

CORPORATE SPORTS UNLIMITED, INC., WHICH PLACED SIX (6) OPEN HAND/GOOD MEASURE MEALS REGISTERED DIETITIANS INTO 15 CORPORATE FITNESS CENTERS TO OFFER NUTRITION SERVICES TO EMPLOYEES AND MEMBERS. GOOD MEASURE MEALS ALSO INITIATED THE RACE FOR GOOD PROGRAM THAT SUPPORTS LOCAL 5K CHARITY RACES BY ENGAGING GMM CUSTOMERS, EMPLOYEES, AND COMMUNITY STAKEHOLDERS TO PARTICIPATE IN PHYSICAL ACTIVITY. IN 2013, OPEN HAND EXTENDED THE REACH OF COMPREHENSIVE NUTRITION CARETM BY ACHIEVING SEVERAL KEY GOALS: A.THROUGH OUR COMMUNITY NUTRITION SERVICES, OPEN HAND SERVED APPROXIMATELY 6,075 CLIENTS IN 2013. THIS INCLUDES MORE THAN 1,422,000 MEALS AND SUPPLEMENTS SERVED TO OVER 3,500 CLIENTS. IN 2013, OUR REGISTERED DIETITIANS WORKED DIRECTLY WITH 4,060 CLIENTS, PROVIDING EXTENSIVE NUTRITION EDUCATION RANGING FROM THE INSTRUCTION ON THE NUTRITIONAL VALUE OF THE FOODS IN THEIR DIET TO CALORIE BALANCE AND DIABETES MANAGEMENT, CONDUCTING HOME NUTRITION ASSESSMENTS, AND PROVIDING IN-CLINIC MEDICAL NUTRITION THERAPY (MNT). B. FINALLY, THE FOLLOWING PROGRAMMATIC MILESTONES WERE ACHIEVED IN 2013: 1.PREPARED MEALS PROGRAM - IN 2013, CLIENTS REPORTED IMPROVEMENTS IN OUTCOMES DIRECTLY OR INDIRECTLY RESULTING FROM THEIR PARTICIPATION IN OPEN HAND'S PREPARED MEALS PROGRAM. 68.6% OF CLIENTS REPORTED THAT THE MEALS HELPED THEM TO MAINTAIN A HEALTHY WEIGHT, COMPARED TO 67.4% IN 2012; 45.7% REPORTED THAT THE MEALS HELPED THEM TO INCREASE THEIR ENERGY LEVELS, COMPARED TO 39.7% IN 2012; 31.1% REPORTED THEIR HEALTHCARE PROVIDERS TOLD THEM THEIR BLOOD SUGAR LEVELS WERE IMPROVED, VS. 29% IN 2012. 2.MARKET BASKET PROGRAM - ADMINISTERED BY OPEN HAND REGISTERED DIETITIANS (RDS), THE MARKET BASKET PROGRAM IS AN INNOVATIVE SOLUTION TO REDUCE FOOD INSECURITY, INCREASE INTAKE OF HEALTHY FOODS, AND INCREASE NUTRITION KNOWLEDGE IN HIGH-RISK ADULTS IN THE ATLANTA AREA. OPEN HAND VOLUNTEERS DELIVER PROGRAM PARTICIPANTS WEEKLY MARKET BASKETS WHICH INCLUDE FRESH FRUIT AND SEASONAL VEGETABLES, ALONG WITH NUTRITION EDUCATION MATERIALS AND RECIPES RELEVANT TO THE CONTENTS OF THAT WEEK'S BASKET TO ENCOURAGE CONSUMPTION. IN 2013, 76.6% PERCENT OF THOSE SERVED INCREASED THEIR FRUIT AND VEGETABLE CONSUMPTION AND 60.2% INCREASED THEIR NUTRITION KNOWLEDGE SCORES. 36 PERCENT REPORTED NEVER, OR RARELY, BEING CONCERNED ABOUT HAVING ENOUGH FOOD WHILE ON THE PROGRAM. 3.CARE TRANSITIONS PROGRAM - TO HELP CREATE A MODEL OF SERVICE THAT WILL REDUCE THE NUMBER OF RE-HOSPITALIZATIONS FOR SENIORS WHO ARE DISCHARGED FROM A HOSPITAL STAY, OPEN HAND SERVED ON A LOCAL COUNTY PLANNING COMMITTEE CHARGED WITH LAUNCHING A "CARE TRANSITIONS" PROGRAM. THE PROGRAM ENSURED THAT SENIORS HAVE VITAL SERVICES IN PLACE, SUCH AS MEDICALLY APPROPRIATE NUTRITION, AS SOON AS THEY ARE RELEASED FROM THE HOSPITAL, THUS REDUCING THE RISK FOR RE-HOSPITALIZATION. THE OUTCOMES DERIVED FROM THIS PROGRAM WILL BE USED TO DEMONSTRATE THE COST BENEFIT TO THE COMMUNITY OF THIS INITIATIVE IN HOPES OF INSPIRING ADDITIONAL FUNDING TO EXPAND THIS PROGRAM. IN 2013, THE NUMBER OF CLIENTS SERVED THROUGH THIS PROGRAM INCREASED FROM 122 TO 379 OVER THE YEAR, A 311% INCREASE. 4.YOUTH AND FAMILIES NUTRITION & MEALS - AS PART OF AN INCREASED FOCUS ON OUTREACH TO YOUTH AND FAMILIES, OPEN HAND DEVELOPED THE HEALTHY BALANCE KIDS MENU WHICH PROVIDES A FRESH FRUIT OR VEGETABLE AS PART OF EVERY MEAL AND EXCEEDS USDA NUTRITIONAL STANDARDS. THE PROGRAM SERVED 50,000 HEALTHY BREAKFASTS, LUNCHES, AND SNACKS TO AN AVERAGE OF 1,100 AT-RISK CHILDREN EACH DAY DURING A SUMMER PROGRAM IN THE ATLANTA-METRO AREA. ADDITIONALLY, OPEN HAND'S REGISTERED DIETITIANS PROVIDED NUTRITION CLASSES FOR THE SUMMER PROGRAM WITH A CURRICULUM THAT FOCUSED ON HEALTHY FOODS AND "EATING THE RAINBOW." 5.CHRONIC DISEASE PREVENTION - THROUGH A HEALTH PROMOTIONS INITIATIVE - BUILT ENVIRONMENT CONTRACT FROM FULTON COUNTY DEPARTMENT OF HEALTH AND WELLNESS, THE OPEN HAND NUTRITION SERVICES TEAM SERVED 251 FULTON COUNTY RESIDENTS WITH A FITNESS AS MEDICINE WALKING PROGRAM. THE WALKING PROGRAM IS FREE FOR ALL AGES AND ALL FITNESS LEVELS FOR THOSE PARTICIPATING IN FULTON COUNTY. 6.FAITH BASED CHRONIC DISEASE TRAINING - OPEN HAND COMPLETED THE STANFORD CHRONIC DISEASE SELF-MANAGEMENT PROGRAM (CDSMP) TRAINING FOR MEMBERS OF THE FAITH-BASED COMMUNITY WITH FUNDING FROM ALLIANT HEALTH SOLUTIONS'. THE FOUR DAY TRAINING WAS OFFERED TO 21 INDIVIDUALS FROM A DIVERSE GROUP OF INTERFAITH PARTICIPANTS, OF WHOM 18 RECEIVED A CERTIFICATE FOR COMPLETING THE PROGRAM. |
