FORM 990, PART III, LINE 4A | HEALTHY LIVING INITIATIVE (CONTINUED) TABLET COMPUTER PROGRAM FOR SENIORS THE HEALTH TRUST HELPS SENIORS AGE WELL IN PLACE BY PROVIDING PROGRAMS THAT REDUCE SENIOR SOCIAL ISOLATION. IN FY 2015, WE LAUNCHED THE FIRST COMPUTER TABLET PROGRAM FOR SENIORS OVER 65 YEARS OLD RESIDING WITHIN SANTA CLARA COUNTY AS A MEMBER OF A NATIONAL PARTNERSHIP WITH THE NATIONAL COUNCIL ON AGING AND FUNDED BY THE VERIZON FOUNDATION. 50 SENIORS WITH NO EXPERIENCE USING TABLET COMPUTERS OR SIMILAR SOURCES OF TECHNOLOGY WERE SCREENED INTO THE PROGRAM. OF THE 50 SENIORS, 25 WERE HOMEBOUND AND 25 WERE NOT. THE HOMEBOUND SENIORS RECEIVED A PERSONAL COACH WHO VISITED THEM IN HOME TO TEACH THEM HOW TO USE THE TABLET COMPUTER. EACH SENIOR WAS GIVEN A PERSONAL TABLET COMPUTER WITH AN ATTACHED INTERNET DATA PLAN. THEY WERE TAUGHT HOW TO USE TABLET COMPUTERS AND HOW TO ENGAGE WITH FRIENDS AND FAMILY ONLINE USING EMAIL, SOCIAL MEDIA AND SKYPE.THEY ALSO LEARNED HOW TO RESEARCH INFORMATION ONLINE TO BECOME BETTER ADVOCATES FOR THEMSELVES AND THEIR HEALTH. THE PLAN IS TO EXTEND THIS PROGRAM INTO FY 2016. |

THE OUTSIDE CONSULTANT REVIEWS COMPARABILITY DATA FROM COMPENSATION STUDIES AND OTHER SOURCES IN ARRIVING AT A RANGE OF REASONABLE COMPENSATION. THE COMPENSATION COMMITTEE REVIEWS THE REPORT AND DATA PROVIDED OUTSIDE CONSULTANT AND DOCUMENTS ITS COMPENSATION DECISIONS ON A CONTEMPORANEOUS BASIS.
