FORM 990, PART VI, SECTION B, LINE 11 | BEFORE THE RETURN IS FILED, COPIES OF THE 990 WILL BE DISTRIBUTED TO THE MEMBERS OF THE EXECUTIVE COMMITTEE, WHICH CURRENTLY INCLUDE THE BOARD PRESIDENT, BOARD TREASURER AND EXECUTIVE DIRECTOR. COPIES WILL BE SENT TO ALL BOARD MEMBERS IF THE EXECUTIVE COMMITTEE SO DIRECTS. THE EXECUTIVE COMMITTEE MEMBERS WILL BE GIVEN ADEQUATE TIME TO REVIEW THE DOCUMENT AND DISCUSS ANY QUESTIONS AND/OR COMMENTS TO THE DIRECTOR OF FINANCE AND ADMINISTRATION. AFTER ALL QUESTIONS AND CONCERNS ARE RESOLVED, THE EXECUTIVE COMMITTEE WILL APPROVE THE 990 RETURN AND SUCH APPROVAL WILL BE RECORDED IN THE MINUTES OF THE NEXT SCHEDULED BOARD MEETING. |
FORM 990, PART VI, SECTION B, LINE 12C | IN POLICY AND PRACTICE THE AGENCY'S MANAGEMENT AND BOARD OF DIRECTORS REMAIN VIGILENT IN ON GOING REVIEW AND AVOIDANCE OF ANY RELATIONSHIPS OR ACTIVITIES WHICH MIGHT PRESENT A CONFLICT OF INTEREST. MANAGEMENT AND DIRECTORS ARE REQUIRED TO AFFIRM ADHERENCE TO THIS POLICY ON AN ANNUAL BASIS. |
