Form 990, Part VI, Section B, line 11	The Board members receive a draft copy of form 990 through the Board portal on the Organization's intranet. Members get an e-mail informing them that the 990 is posted. They are asked to review, provide input, and approve prior to filing.
Form 990, Part VI, Section B, line 12c	All employees are required to report when an actual or potential conflict of interest occurs. The Organization has established a policy which requires all Board members and key employees to sign a conflict of interest statement annually. Conflict of interest disclosures are reviewed by the CEO and Board Chair.
Form 990, Part VI, Section B, line 15	The Organization has a formal salary administration plan that is reviewed in detail on a three year cycle. All positions, including the CEO and other top management positions are administered under this formal plan. The Board of Directors approves any compensation changes for the CEO based on accomplished goals established on an annual basis. The CEO evaluates key management position performance, and within the guidelines of the formal plan, determines salary changes. An independent consultant reviews the local and regional job market for comparable positions in similar organizations and performs a detailed analysis and recommendation every three years. A mini review is undertaken on an annual basis.
