PART I, LINE 2: GRANTEES' ELIGIBILITY IS DETERMINED BY A THIRD-PARTY FIRM THAT ASSESSES FINANCIAL NEED. ALL APPLICANTS MUST SEND TAX RECORDS, PUBLIC HOUSING ASSISTANCE INFORMATION, FOOD STAMP INFORMATION, CHILD SUPPORT, ETC. WHEN THEY SUBMIT SCHOLARSHIP APPLICATION. FOR MONITORING GRANT FUNDS DISTRIBUTED IN THE UNITED STATES THE ORGANIZATION REQUIRES PARTICIPATING SCHOOLS TO REVIEW TWO REPORTS, THE PRINCIPAL ACKNOWLEDGEMENT FORM AND THE SCHOOL VERIFICATION REPORT, PRIOR TO PROVIDING ASSISTANCE CHECKS THREE TIMES EACH YEAR. THESE REPORTS ENSURE THAT THE CHILDREN ARE STILL ENROLLED IN THE SCHOOL AND THAT THERE HAVE BEEN NO CHANGES IN THEIR FINANCIAL STATUS. IN ADDITION, THE ORGANIZATION ALSO REQUIRES SCHOOLS TO SEND REPORT CARDS AND STANDARDIZED TEST SCORES AT THE END OF THE SCHOOL YEAR FOR ALL STUDENTS RECEIVING GRANTS. THE FINAL SCHOLARSHIP PAYMENT IS NOT SENT UNTIL THOSE RECORDS HAVE BEEN RECEIVED. AT ANY TIME, IF REQUESTED, THE COMPLETE RECORDS OF THE SCHOOLS RECEIVING ASSISTANCE MUST BE MADE AVAILABLE TO SCHOOL CHOICE SCHOLARSHIPS, INC. FOR ANY FURTHER ASSISTANCE TO CONTINUE.
