Form 990, Part VI, Section B, Line 11b | The Form 990 is prepared annually by the Organization's outsourced accountant. Upon completion, a draft of the return is forwarded to the Organization's audit firm, the Executive Director, and the Board of Directors. All parties have the opportunity to ask questions and raise comments about any particular part, line item or figure contained in the return. Once all questions and comments have been addressed, the return is finalized and filed electronically. |
Form 990, Part VI, Section B, Line 12c | Each director, principal officer, and a member of a committee with Board delegated powers shall annually sign a statement which affirms such person has received a copy of the conflict of interest policy, has read and understood the policy, has agreed to comply with the policy and understands the Organization is charitable and in order to maintain its federal tax exemption, must engage primarily in activities which accomplish one or more of its exempt purposes. |
Form 990, Part VI, Section B, Line 15 | The Incubator's Board members meet periodically throughout the year to discuss the Executive Director's salary and any performance bonuses. Members consider comparable salaries in Philadelphia and the surrounding region as well as the director's qualifications and work achievements. Salary information is documented and signed by both the Executive Director and the Board. |
Form 990, Part VI, Section C, Line 19 | All governing documents, financial statements, policies and tax returns are maintained at the Organization's office and are available upon request. Requests will be honored within ten (10) business days at no cost to the requester. |
