Form 990, Part VI, Line 6: Explanation of Classes of Members or Shareholder Full Membership - any person or entity which provides and/or coordinates services and/or supports for one or more persons with disabilities that is not owned or operated by a public entity.Associate Membership available to parents, Direct Support Professionals, staff and friends of agencies that are full members of ANCOR, consultants, vendors, suppliers, and other related to - but not providing support or services to persons with disabilities or to public agencies providing services to people with disabilities. An Associate is entitled to all membership benefits excluding the right to vote or hold office in ANCOR.State Provider Association Membership - available to membership organizations like ANCOR that operate on a state level. A state provider association associate member is entitled to all membership benefits in the American Network of Community Options and Resources, including a subscription to the E-LINKS publication, but excluding the right to individually vote in elections. In addition, the state association executive or designee is invited to participate in the ANCOR State Provider Association Executives Forum. The State Provider Association Executives Forum elects one individual to serve as its voting representative on the Board of Directors.
Form 990, Part VI, Line 7b: Describe Decisions of Governing Body Approval by Members or Shareholders Changes in the by laws require a two-thirds (2/3) majority vote of the full members actually participating in the vote, provided that a quorum has been achieved.
Form 990, Part VI, Line 11b: Form 990 Review Process The Form 990 is prepared by our outside CPA with a draft provided to the Director of Finance. The Director of Finance and the Support Accountant review the Form 990 for completeness and accuracy. They reconcile the Form 990 back to their general ledger. The Form 990 is then reviewed by the Board of Directors and the CEO who signs the return upon acceptance. The Finance Committee is updated on the status of the 990 filing and is provided a copy when it is approved.
Form 990, Part VI, Line 15b: Compensation Review and Approval Process for Officers and Key Employees Staff compensation is determined by the CEO and is a budget item for the board of directors' approval. The compensation for the CEO is determined by a designated board of directors' committee. Criteria used is comparable compensation for similar position. An employment agreement is executed between the CEO and ANCOR.
Form 990, Part VI, Line 19: Other Organization Documents Publicly Available ANCOR provides its Policy & Procedures Manual, position statements, and three most recent Form 990 filings and audited financial statements on their website. There are no restrictions on the accessibility of these documents.
