FORM 990, PART III, LINE 2 FAMILY SERVICE OF RHODE ISLAND (FSRI) IS A COMPREHENSIVE NON-PROFIT SOCIAL SERVICE AGENCY THAT IS AN ESSENTIAL PART OF THE RHODE ISLAND COMMUNITY. FSRI PROVIDES SUPPORT TO CHILDREN AND FAMILIES-FROM BIRTH TO ADULTHOOD-THROUGH SERVICES FOCUSED ON THREE GENERAL AREAS--PHYSICAL AND MENTAL HEALTH, EDUCATION, AND OVERALL WELL BEING. EVERY PROGRAM OR SERVICE FSRI OFFERS IS GEARED TOWARDS PROMOTING HEALTHY FAMILIES AND STRONG COMMUNITIES. THE 2016-2017 BROUGHT NEW PROGRAMS AND INITIATIVES TO FSRI: RESOURCE FAMILY SUPPORT SERVICE IS DESIGNED TO ENHANCE RESOURCE PARENTS' ABILITY TO PROVIDE FOR THE SAFETY AND WELL-BEING OF EACH CHILD IN THEIR CARE. THE SERVICE PROVIDES PARENT SKILL-BUILDING, ACCESS TO AN ARRAY OF PERSONALIZED RESOURCES, AND COMMUNITY SUPPORT FOR THE WHOLE FAMILY AND IS DESIGNED TO ENSURE THAT MORE RESOURCE PARENTS ARE LICENSED AND RETAINED OVER TIME, INCREASE THEIR UNDERSTANDING OF CHILDHOOD TRAUMA, AND OFFER PERMANENCY FOR FOSTER CHILDREN. COMMUNITY SERVICES HAS LAUNCHED FUNCTIONAL FAMILY THERAPY, AN EVIDENCE BASED, SHORT-TERM TREATMENT THAT FOCUSES ON THE IMPROVED FAMILY INTERACTIONS TO DECREASE THE CHILD'S BEHAVIORAL PROBLEMS. THE TARGET POPULATION IS ADOLESCENTS AGES 10-19 AND THEIR FAMILIES DELIVERED IN THE OFFICE, HOME, AND COMMUNITY. CURRENTLY, FSRI HAS THREE CLINICIANS WHO ADMINISTER THE PROGRAM IN TWO LANGUAGES AND HAS SEEN SUCCESSFUL OUTCOMES IN TREATMENT. FSRI IS PLANNING TO PILOT THE EVIDENCE-BASED INTERVENTION, FAMILIAS UNIDAS, WHICH IS DESIGNED TO HELP LATINO FAMILIES ADDRESS ISSUES RELATED TO SUBSTANCE ABUSE AND HIGH RISK SEXUAL BEHAVIOR AMONG ADOLESCENTS AND YOUNG ADULTS. FSRI HAS LAUNCHED A PROGRAM WHERE STAFF WILL WORK AS PART OF A COMPREHENSIVE HEALTHCARE TEAM THAT CAN ADDRESS SOCIAL DETERMINANTS OF HEALTH THAT NEGATIVELY IMPACT CHILDREN AND FAMILIES. THE PROGRAM PROVIDES THE OPPORTUNITY TO ADDRESS THE NEEDS OF PEOPLE IN RHODE ISLAND WHO EXPERIENCE FRAGMENTED, UNCOORDINATED HEALTH CARE AND/OR BEHAVIORAL HEALTH CARE. IN PARTICULAR, THE NEEDS OF PATIENTS CONSIDERED HIGH RISK AND HIGH COST, PATIENTS WHO ARE IN THE TOP 5% OF HEALTH UTILIZATION. OUR VISION IS TO HAVE ON-THE-SPOT ACCESS FROM KNOWLEDGEABLE STAFF WHO WILL WORK CLOSELY WITH THE MEDICAL TEAM TO ENSURE THE BEST OUTCOME. FSRI'S NATIONALLY RECOGNIZED EXPERTISE ON CHILD AND FAMILY TRAUMA ISSUES RESULTED IN THE LAUNCH OF EYE MOVEMENT DESENSITIZATION AND REPROCESSING PROGRAM, AN EVIDENCE BASED PRACTICE, THAT SERVICES CLIENTS WHO HAVE EXPERIENCED TRAUMATIC EVENTS.
FORM 990, PART VI, SECTION B, LINE 11B THE FORM 990 IS PREPARED BY THE EXTERNAL AUDITORS AND REVIEWED BY MANAGEMENT. ONCE ALL ARE SATISFIED WITH THE FORM, IT IS FINALIZED AND A COPY IS SENT TO ALL BOARD MEMBERS. AT THE SUBSEQUENT BOARD MEETING, THE BOARD IS ASKED IF THEY HAVE ANY QUESTIONS OR COMMENTS AND A GENERAL REVIEW OF THE FORM IS CONDUCTED BY MANAGEMENT. REVISIONS, CORRECTIONS, ETC ARE MADE AS NECESSARY. SUBSEQUENT TO THIS MEETING THE FORM IS SUBMITTED TO THE IRS
FORM 990, PART VI, SECTION B, LINE 12C THE BOARD OF DIRECTORS AND MEMBERS OF MANAGEMENT COMPLETE CONFLICT OF INTEREST FORMS ON AN ANNUAL BASIS. NEW MEMBERS OF THE BOARD OF DIRECTORS AND ALL NEW EMPLOYEES COMPLETE THE FORM WHEN THEY ARE ELECTED OR IN THE CASE OF EMPLOYEES, HIRED. FORMS ARE REVIEWED AND ANY POTENTIAL CONFLICT IS REVIEWED BY THE BOARD PRESIDENT IN THE CASE OF DIRECTORS OR THE VP OF HUMAN RESOURCES FOR EMPLOYEES. ANY IDENTIFIED CONFLICTS ARE RESOLVED IN ACCORDANCE WITH THE CONFLICT OF INTEREST POLICY.
FORM 990, PART VI, SECTION B, LINE 15A ANNUALLY THE EXECUTIVE COMMITTEE OF THE BOARD ACTING AS A COMPENSATION COMMITTEE CONDUCTS A PERFORMANCE REVIEW AND EVALUATION OF THE CHIEF EXECUTIVE OFFICER. THE REVIEW ALSO ESTABLISHES THE INDIVIDUAL'S COMPENSATAION FOR THE FOLLOWING YEAR. THIS PROCESS INVOLVES THE EVALUATION OF THE INDIVIDUAL AND A REVIEW OF COMPENSATION OR COMPARABLE POSITIONS OBTAINED FROM COMPENSATION SURVEYS. THE BOARD'S DELIBERATION AND DECISION IS NOTED IN THE MINUTES OF THE MEETING.
