FORM 990, PART VI, SECTION A, LINE 7A EACH MEMBER COMPANY IS ENTITLED TO APPOINT ONE MEMBER TO THE BOARD OF DIRECTORS.
FORM 990, PART VI, SECTION B, LINE 11 THE ACCOUNTING FIRM PREPARER EMAILS A COPY OF THE FORM 990 TO THE BOOKKEEPER. THE BOOKKEEPER REVIEWS THE RETURN WITH THE EXECUTIVE DIRECTOR FOR ANY ERRORS. THE EXECUTIVE DIRECTOR THEN PRESENTS THE TAX RETURN TO THE EXECUTIVE COMMITTEE FOR THEIR APPROVAL DURING THE BOARD MEETING.
FORM 990, PART VI, SECTION B, LINE 15 AT ACTA, EACH POSITION HAS STANDARDS THAT SET FORTH THE EXPECTATIONS FOR PERFORMANCE. THESE EXPECTATIONS ARE IDENTIFIED IN THE JOB DESCRIPTIONS AS WELL AS IDENTIFIED THROUGH YOUR JOB OBJECTIVES WHICH ARE AGREED UPON DURING THE PRIOR PERFORMANCE REVIEW. EACH EMPLOYEE SHALL HAVE A PERFORMANCE REVIEW THREE MONTHS AFTER COMMENCING EMPLOYMENT AT ACTA AND AT LEAST ANNUALLY THEREAFTER ON THE ANNIVERSARY OF THE EMPLOYEE'S START DATE TO DISCUSS PERFORMANCE AND/OR ACCOMPLISHMENTS AND TO REACH AN UNDERSTANDING ON FUTURE GOALS AND PERFORMANCE EXPECTATIONS. THE FINAL DECISION FOR STAFF SALARY ADJUSTMENTS WILL BE MADE BY THE EXECUTIVE DIRECTOR AFTER TAKING INTO ACCOUNT PERFORMANCE AS WELL AS THE FINANCIAL CONDITION OF THE ORGANIZATION. THE FOUNDATION OF SUCCESSFUL PERFORMANCE IS OPEN, TWO-WAY COMMUNICATION. THE EMPLOYEE PRE-EVALUATION DATA SHEET AND REVIEW FORM WILL BE AN IMPORTANT PART OF THE REVIEW DISCUSSION AS IT WILL BE USED TO DOCUMENT PERFORMANCE LEVEL. THE EMPLOYEE WILL BE INFORMED IF PERFORMANCE IS CONSIDERED AT, ABOVE, OR BELOW EXPECTATIONS. THEY WILL HAVE AN OPPORTUNITY TO MAKE ANY COMMENTS ON THE REVIEW THEY FEEL ARE APPROPRIATE AND SUCH COMMENTS WILL BE INCLUDED IN THEIR PERSONNEL FILE. IN AREAS WHERE PERFORMANCE IS DEFICIENT AND IN NEED OF IMPROVEMENT, THE EMPLOYEE WILL WORK WITH THE MANAGER TO DEVELOP AN ACTION PLAN DESIGNED TO HELP ELEVATE THEIR PERFORMANCE. IF ACCEPTABLE RESULTS ARE NOT ACHIEVED, THE CONSEQUENCES OF CONTINUED BELOW-STANDARD PERFORMANCE WILL BE DISCUSSED. THE CONSEQUENCES, DEPENDING ON THE CIRCUMSTANCES, MAY VARY, UP TO AND INCLUDING TERMINATION OF EMPLOYMENT. IT WILL BE THE RESPONSIBILITY OF THE PERSONNEL COMMITTEE TO CONDUCT AN ANNUAL REVIEW OF THE EXECUTIVE DIRECTOR'S PERFORMANCE WHICH WOULD BE EFFECTIVE AT THE START OF THE NEW FISCAL YEAR (OCTOBER 1ST) AND TO RECOMMEND SALARY ADJUSTMENTS TO THE EXECUTIVE COMMITTEE WHICH WILL MAKE THE FINAL DECISION BASED ON PERFORMANCE AND THE FINANCIAL CONDITION OF THE ORGANIZATION. THE FINAL DECISION FOR SALARY ADJUSTMENTS WILL BE MADE BY ACTA'S EXECUTIVE COMMITTEE AFTER TAKING INTO ACCOUNT THE FINANCIAL CONDITION OF THE ORGANIZATION. REVIEW AND APPROVAL PROCESS FOR OTHER OFFICERS AND KEY EMPLOYEES FOLLOWS THE SAME PROCEDURES AS THE PROCESS FOR TOP MANAGEMENT.
