CORE FORM, PART III; STMT OF PROGRAM SERVICE ACCOMPLISHMENTS COMMUNITY BENEFIT STATEMENT =========================== Hackensack Meridian Health, Inc. is a non-profit health care organization and is the sole corporate member of various health care-related organizations, the majority of which are tax-exempt entities. The Internal Revenue Service ("IRS") has recognized Hackensack Meridian Health, Inc. as being a tax-exempt organization under Internal Revenue Code ("IRC") Section 501(c)(3). As the central organization in the group ruling of the tax-exempt entities included in this group tax return, Hackensack Meridian Health, Inc. strives to continually develop and operate a multi-hospital integrated health care delivery network which provides a comprehensive spectrum of medically necessary health care services to the residents of New Jersey. Hackensack Meridian Health, Inc. ensures that its network provides medically necessary health care services to all individuals regardless of race, color, creed, sex, national origin, religion or ability to pay. The Network operates an extensive acute care hospital system which consists of two academic medical centers (which include two childrens hospitals and a cancer center), and seven community hospitals. The following hospitals are included in this group tax return: - Jersey Shore University Medical Center, located in Neptune, New Jersey, is a major academic medical center and regional trauma center with 614 beds that includes the K. Hovnanian Childrens Hospital(1); - Riverview Medical Center is a 468-bed community hospital located in Red Bank, New Jersey(1); - Raritan Bay Medical Center at Perth Amboy is a 395-bed community hospital located in Perth Amboy, New Jersey; - Ocean Medical Center is a 281-bed community hospital located in Brick, New Jersey(1); - Bayshore Community Hospital is a 211-bed community hospital located in Holmdel, New Jersey(1); - Palisades Medical Center, located in North Bergen, New Jersey, is a 206-bed community hospital, that includes a 247-bed nursing home known as the Harborage; - Southern Ocean Medical Center is a 176-bed community hospital located in Manahawkin, New Jersey(1); and - Raritan Bay Medical Center Old Bridge, located in Old Bridge, New Jersey, is a 113-bed community hospital. (1) These hospitals are divisions of Meridian Hospitals Corporation. All of these hospitals are recognized by the IRS as IRC Section 501(c)(3) tax-exempt organizations. Pursuant to their charitable purposes, the hospitals provide medically necessary health care services to all individuals in a non-discriminatory manner regardless of race, color, creed, sex, national origin religion or ability to pay. Moreover, the hospitals operate consistently with the following criteria outlined in IRS Revenue Ruling 69-545: 1) each hospital provides medically necessary health care services to all individuals regardless of ability to pay, including charity care, self-pay, Medicare and Medicaid patients; 2) each hospital operates an active emergency department for all persons; which is open 24 hours a day, 7 days a week, 365 days per year; 3) each hospital maintains an open medical staff, with privileges available to all qualified physicians; 4) control of each hospital rests with its board of directors; which is comprised of independent civic leaders and other prominent members of the community; and 5) surplus funds are used to improve the quality of patient care, expand and renovate facilities and advance medical care, programs and activities. By harnessing the collective knowledge and resources of both Hackensack University Health Network and Meridian Health, we have created a world-class health network. The physicians and caregivers from Hackensack Meridian Health are among the finest in the nation streamlining care, putting their hearts and minds into the care they provide, offering patients more options and discovering and innovating for tomorrow. Hackensack Meridian Health combines the excellence and innovation of academic medical centers with the convenience and compassion of community-based care and services. The network consists of 13 hospitals, including two academic medical centers, two childrens hospitals, nine acute care hospitals, physician practices, more than 120 ambulatory care centers, surgery centers, home health services, long-term care and assisted living communities, ambulance services, lifesaving air medical transportation, fitness and wellness centers, rehabilitation centers and urgent care and after-hours centers. Hackensack Meridian Health also trains tomorrows doctors and allied health professionals and conducts significant research that results in new ways of preventing and treating disease. High on the list of milestones will be the opening of the only private school of medicine in New Jersey, in conjunction with Seton Hall University, to further punctuate Hackensack Meridian Healths focus on academic excellence. The partnership will also include nursing and allied health sciences. By combining and sharing resources and identifying efficiencies, Hackensack Meridian Health is providing patients with the highest quality care at the most appropriate cost, meeting the needs of the larger communities it serves and enhancing its ability to be innovative in the delivery of care. Behavioral Health ================= Behavioral Health aligns a broad range of specialty behavioral health services across the network and centers of excellence in areas such as mood disorders, child and adolescent behavioral health disorders, integrated medical and behavioral health care, geriatric psychiatry and substance use disorders. Hackensack Meridian Healths long history of community outreach and prevention services combined with recent population health advances and initiatives will help to position Behavioral Health well for future growth. With the formation of our new organization, Hackensack Meridian Behavioral Health became one of the largest and most comprehensive providers of Behavioral Health services and programs in the state, poised with 165 inpatient psychiatry beds and 10 outpatient centers. Additionally, with Emergency Psychiatric Services at each of our hospital Emergency Departments, Hackensack Meridian Behavioral Health will continue its leadership role as an organization with a core commitment to meeting the Behavioral Health needs of the communities we serve. The Outpatient Counseling Center at Palisades Medical Center has crafted its services to meet the needs of their bilingual (Spanish/English) community, where many services are provided by bilingual psychiatrists and counselors. Providing these services and meeting the demand for bilingual Behavioral Health practitioners is one of the greatest challenges faced by Behavioral Health organizations. Palisades Medical Center has made meeting this need a strategic priority for serving their community. At Jersey Shore University Medical Center, physician leaders, clinicians, and members of the community have joined forces to address the growing problem of substance abuse and the need for additional behavioral health services within our community. Jersey Shore received a $950,000 grant from the NJ Department of Health for the Community Engagement, Screening, Assessment and Referral (CESAR) program. To date, more than 14,000 screenings have taken place. Addressing the Drug Epidemic ---------------------------- In response to the heroin and opiate abuse epidemic affecting the nation, and particularly Monmouth and Ocean counties, Scientific Magazine hosted a forum addressing the issue with experts offering four perspectives, including Government, Criminal Justice, Medicine and Industry. The event was held at Jersey Shore University Medical Center in May. New Jerseys Governor also visited infants facing perinatal addiction in Jersey Shores NICU. The visit reinforced how even the youngest, most innocent members of our community are affected by addiction. Jersey Shore University Medical Center has been actively involved in supporting first responder efforts out in the community. Providing law enforcement officials with replacement naloxone kits helps the police departments manage rising costs while ensuring that the life-saving antidote for heroin and opioid overdoses is available when minutes count.
CORE FORM, PART III - CONTINUED Pediatric Behavioral Health --------------------------- The Pediatric Psychiatry Collaborative Grant was awarded an expansion by the NJ Department of Children and Family Services allowing Hackensack Meridian Health and Cooper Health to expand its current services into seven additional counties in conjunction with a new grant partner, St. Peters Health. The Pediatric Psychiatry Collaborative Pilot Program (PPC) is a state funded grant program that partners Hackensack Meridian Health and Cooper University Health System to provide support for children with behavioral and emotional health issues. The program is available for children up to age 18 and requires the childs primary care provider (PCP) to universally offer standardized behavioral health screenings at each childs well visit. Hackensack Meridian Health & Cooper Health each act as a hub whereby the PCP can make a referral for access to child mental health services. Combined, the hubs are currently serving eleven counties in NJ. Behavioral Health Home Program ------------------------------ Behavioral Health gained certification as a Behavioral Health Home, which is a Medicaid State Plan option that provides an opportunity to build a person-centered system of care that achieves improved outcomes and better services and value for the NJ Medicaid program. A Behavioral Health Home allows individuals to have all of their health care needs identified, addressed, and treated in a coordinated way. It is expected that the use of a behavioral health home model will result in improved health outcomes for the consumer base, better quality of treatment, and improved cost effectiveness; improved consumer experience with care; and declines in the use of hospitals, emergency departments, and other costly inpatient care. Advanced Care for Depression ---------------------------- Jersey Shore University Medical Center is the first hospital in New Jersey to offer deep transcranial magnetic stimulation (dTMS) a non-invasive treatment for depression that provides new hope for patients who have experienced side effects from medications or for whom medications have proven ineffective. dTMS is a non-invasive technique that uses magnetic pulses to painlessly penetrate the cranium and target the prefrontal cortex of the brain, where mood is regulated. It works by stimulating neurons responsible for depressive symptoms and restoring neuronal conduction, along with equilibrium of brain chemicals, in the neural pathways of the brain. The outpatient treatment comprises of 20-minute sessions with almost no side effects, and does not disrupt a patients daily activity. Most patients experience improvement after four to six weeks of treatment. Cancer Care =========== Hackensack Meridian Health offers a comprehensive range of oncology services across seven of our hospitals. Together, this broad network of services brings together clinical experts, Phase I research, advanced technology focused on positive outcomes in the following cancers: breast, gastrointestinal, genitourinary, gynecological, head and neck, leukemia, lymphoma, multiple myeloma, neuro-oncology, skin and sarcoma, hepatobiliary, endocrine and thoracic. In December, at the site of the forthcoming Seton Hall-Hackensack Meridian School of Medicine, Memorial Sloan Kettering Cancer Center and Hackensack Meridian Health announced the Memorial Sloan KetteringHackensack Meridian Health 10-year strategic partnership. This partnership brings together two world-class organizations with deep New Jersey roots to foster a single goal: Finding more cures for cancer faster while ensuring that patients have access to the highest-quality, most individualized cancer care when and where they need it. To achieve this goal, the new Memorial Sloan Kettering Cancer CenterHackensack Meridian Health partnership will combine both organizations unparalleled expertise in all areas of cancer care and research, as well as their commitment to excellence, to accelerate new discoveries and improve the lives of patients they jointly serve, while training a new generation of leaders in the field. As a result of this partnership, patients will now have access to a broader portfolio of hundreds of clinical trials - including phase I trials - that both institutions offer. Together, Hackensack Meridian Health and Memorial Sloane Kettering Cancer Center will enhance the capabilities of each other, offering cancer patients access to more than 1,000 clinical trials. Combined they will perform more than 900 bone marrow transplants, one of the largest programs in the world. According to the American Cancer Society, New Jersey has a higher cancer incidence rate than the national average. The Memorial Sloan Kettering Cancer Center Hackensack Meridian Health partnership aims to improve the state of cancer care in New Jersey and beyond by providing support to patients throughout the cancer care continuum, from diagnosis through active treatment and into survivorship. Patients will benefit from greater access to the most innovative concepts and treatments in cancer care, including precision medicine, immunotherapy, and cell-based therapies. Each organizations existing sites of care in New Jersey - including Hackensack Meridian Healths cancer care locations, John Theurer Cancer Center at Hackensack University Medical Center, and Memorial Sloan Ketterings locations in Basking Ridge, Middletown, and Montvale (opening in 2018) - will be part of the Memorial Sloan Kettering Cancer Center Hackensack Meridian Health partnership, but each organization will independently own, operate and manage these existing sites. The initial stages of the Memorial Sloan Kettering Cancer Center Hackensack Meridian Health partnership will involve developing joint standards of care to be applied across these existing sites to optimize clinical outcomes for every patient, as well as setting clinical research priorities. Eventually, the two organizations will create a formal joint venture to own and operate new ambulatory care centers in areas of New Jersey they do not yet serve. Our combined and unparalleled clinical experience, along with our focus on big-data platforms, next-generation analytics and precision medicine, will lead to the best possible outcomes for patients. Together we will develop and implement standardized care management and research protocols for all cancer patients receiving care at Memorial Sloan Kettering Cancer Center and Hackensack Meridian Health partnership sites. Memorial Sloan Kettering Cancer Center is deeply committed to New Jersey, having provided cancer care in the state for more than 20 years. Today, Memorial Sloan Kettering Cancer Center and Hackensack Meridian Health treat one in five New Jersey residents who are diagnosed with cancer. Combined, we will annually serve the most patients with cancer in the region, and many of their combined programs will be among the largest and most respected in the country. The Memorial Sloan Kettering Cancer Center Hackensack Meridian Health partnership creates instant convenience. All Memorial Sloan Kettering Cancer Center locations in New Jersey (in Basking Ridge, Middletown and Montvale) and Hackensack Meridian Health locations that offer cancer care will participate in the partnership. Physicians employed by Memorial Sloan Kettering Cancer Center and Hackensack Meridian Health - including the Hackensack Division of Regional Cancer Care Associates and physicians who participate in Hackensack Meridian Healths clinically integrated network - will continue to care for patients at their respective sites. The partnership will be overseen by an Operating Board made up of representatives from each organization. That board will function with the advice of a formal clinical council, led by internationally recognized experts in all subtypes of cancer from Memorial Sloan Kettering Cancer Center, John Theurer Cancer Center and Hackensack Meridian Health, as well as a formal executive advisory group comprising institutional leaders in key areas.
CORE FORM, PART III - CONTINUED Offering Residents a New Healing Outpatient Experience ------------------------------------------------------ Construction of a 299,000-square-foot facility began in November of 2015 at Jersey Shore University Medical Center. This facility will become the HOPE Tower a $265 million building development on the hospitals east campus that will provide a new healing outpatient experience for the community. Guided by a patient-centered approach towards care and informed by the latest medical breakthroughs, HOPE Tower expands upon Jersey Shores comprehensive services by providing a thoughtful and accessible health care experience. Two floors within HOPE Tower will house a 58,000-square-foot Cancer Care center that will offer comprehensive treatment options, including surgical specialties, medical oncology, advanced radiation therapy and minimally invasive interventional therapies. The new center is designed for Jersey Shores multidisciplinary team of specialists who deliver coordinated and compassionate care treating a range of oncological conditions. Jersey Shore is the regions only academic university-level teaching center, and HOPE Tower will serve as home to medical education programs, research services and faculty physician offices in medical practice areas including Surgery, Medicine, Pediatrics, Psychiatry and Obstetrics and Gynecology. With expert clinicians, a state-of-the-art amphitheater and a high-tech simulation laboratory, HOPE Tower will foster the next generation of medical leaders and provide access to the most promising clinical trials and therapies. Reaching Milestones ------------------- 2016 marked the opening of cancer care centers at three of the network hospitals including Southern Ocean Medical Center in September, Riverview Medical Center in November and Ocean Medical Center in November. All three facilities held opening events to which they invited community members to tour the facilities and hear about the new services that will be available to them. Cardiovascular Care =================== Hackensack Meridian Health is a recognized leader for cardiovascular clinical excellence and research. The cardiovascular programs at Hackensack Meridian Health continually achieve outstanding patient outcomes by offering the latest medical advancements to diagnose and treat virtually all heart and vascular conditions. Participating in more than 65 clinical research trials, Hackensack Meridian Health provides patients access to the most innovative technologies and new medical therapies available. We are also committed to furthering our goal to educate the next generation of cardiologists. Jersey Shore University Medical Center recently received accreditation for its new cardiovascular fellowship program, which provides comprehensive training in clinical cardiology as well as in-depth exposure to cardiovascular subspecialties and research. Additionally, Hackensack Meridian Healths Transcatheter Aortic Valve Replacement (TAVR) program continues to grow at Jersey Shore University Medical Center, providing patients throughout the region with superior outcomes to treat aortic stenosis. Collectively, Hackensack Meridian performed 214 TAVR procedures in 2016 and expects volumes to continue to increase as the FDA recently approved the use of the procedure on intermediate-risk patients. Patients living with valve disorders benefit from a strong network of cardiovascular experts who take a multidisciplinary approach using the newest techniques to ensure the very best outcomes. In 2016, Jersey Shore became the first hospital in New Jersey to implant the Micra Transcatheter Pacing System (TPS) the worlds smallest pacemaker since the device gained U.S. Food and Drug Administration (FDA) approval in April 2016. Micra TPS is a new type of heart device that treats patients with bradycardia, a common heart condition characterized by a slow or irregular heart rhythm. At one-tenth the size of a traditional pacemaker, Micra TPS is the only leadless pacemaker approved for use in the U.S. The minimally invasive procedure takes less than an hour, and unlike traditional pacemakers, Micro TPS is not visible. Achievements ------------ Hackensack Meridian Health has plenty of heart when it comes to providing patients with the highest level of coordinated and personalized cardiovascular care by our extensive network of heart and vascular specialists at every patient touch point, including at our award-winning hospitals. CORE FORM, PART III - CONTINUED Neuroscience ============ Hackensack Meridian Health offers a comprehensive range of neuroscience services at seven of our hospitals, including The Neuroscience Institute at Jersey Shore University Medical Center. Together, this broad network of services brings together clinical experts, advanced technology, innovative treatments and patient-centered care to manage and treat conditions of the brain, spine and nervous system. Staffed by neurologists, neurosurgeons (pediatric and adult), researchers and specialists in neuro-immunology, neuro-oncology, neuropsychology, movement disorders and neuropathy, this team takes an interdisciplinary approach to care. In 2016, Jersey Shore University Medical Center was recognized by The Joint Commission and the American Heart Association/American Stroke Association as meeting The Joint Commissions standards for Disease-Specific Care Comprehensive Stroke Center Certification, which means it is part of an elite group of providers focused on complex stroke care. Complex Stroke Centers are recognized as industry leaders and are responsible for setting the national agenda in highly-specialized stroke care. Comprehensive Stroke Center Certification recognizes those hospitals that have state-of-the-art infrastructure, staff and training to receive and treat patients with the most complex strokes. Jersey Shore underwent a rigorous onsite review in May 2016. Joint Commission experts reviewed Jersey Shores compliance with the Comprehensive Stroke Center standards and requirements including advanced imaging capabilities, 24/7 availability of specialized treatments, and staff with the unique education and competencies to care for complex stroke patients. 2016 saw the highest volume of neurosurgical procedures to date, network-wide. And, over the past five years, volume has doubled at The Neuroscience Institute at Jersey Shore University Medical Center. To support this surgical volume, Jersey Shores Neuro-ICU provides specialized, comprehensive care for severe neurological and neurosurgical conditions, and access to the regions only designated comprehensive Stroke Rescue Center and Level II Trauma Center. Staffed with Magnet nurses and other specialists, the Institute is leading the way. Additionally this year, weve expanded our services for pediatric neurosurgery. The ALS Center launched at Jersey Shore University Medical Center this year and is the only program dedicated to treating ALS in Monmouth and Ocean counties. Its collaboration with the ALS Association (ALSA) allows the center to exchange information and best-care practices with the nations leading physician experts and clinics helping to ensure that people living with ALS in our region have access to specialized care. The center is also fortunate to have the support of the Joan Dancy and PALS Foundation, which provides valuable feedback, education and in-servicing to enhance the care experience. The foundations see the Hackensack Meridian ALS Center as a program that supports their mission; thus, the foundations are able to provide consultative support to the center. Stroke Care ----------- The integration of technology into patient care has become an important part of health care. Launched throughout Monmouth and Ocean counties in 2016, the Telestroke Network ensures the best possible outcomes for all of our patients by enhancing the availability of stroke expertise when it is needed most. In Telestroke, a "robot" enables suspected stroke patients to be seen by a board-certified neurologist within minutes of arrival at the Emergency Department when every minute counts. By using videoconferencing and image sharing technology, a Telestroke neurologist, who can be located anywhere in the U.S., can examine a patient, assess them for stroke symptoms, directly communicate with them, and rapidly prescribe a treatment such as tissue plasminogen activator (tPA), which can help to reverse the disability of certain types of stroke if given within the first three hours. In addition to being able to zoom in on the patient and communicate directly with them, their family members, and the other patient care providers, Telestroke enables the physician to order and review tests to better determine what type of stroke the patient is experiencing and immediately recommend a treatment option. Patients are now receiving faster care, which leads to improved outcomes. In fact, the tPA administration rate saw a significant increase. Prior to the launch, the system rate was 11 percent; after the launch, the rate grew to 21.25 percent across the network. Childrens Care =============== By establishing Hackensack Meridian Health as a network in 2016, our sub-specialized pediatric services are reaching even more communities throughout the region. This year, a strategic leadership team was initiated to work collaboratively in identifying the ways which Hackensack Meridian Health can continue to improve access to quality pediatric specialty care across the state to best serve our patient population. Comprised of key leaders from Joseph M. Sanzari Childrens Hospital and K. Hovnanian Childrens Hospital, the team is building the foundation for a seamless and coordinated pediatric continuum of care. This includes the expansion of pediatric nephrology, pediatric rheumatology and pediatric hematology/oncology experts from Joseph M. Sanzari Childrens Hospital, connecting them with patients across the network, and working in sync with pediatricians and subspecialists at K. Hovnanian Childrens Hospital to make these services accessible to families of Monmouth and Ocean counties. In 2016, pediatric services across Hackensack Meridian Health continued to grow, with specialized programs established at our two Childrens Hospitals to address the unique needs of children. These services include Pediatric Sleep and Pediatric Palliative Care programs at K. Hovnanian Childrens Hospital, plus Integrative Medicine, Adolescent Medicine, and The Joint Commission-certified Pediatric Pain and Palliative Care program at Joseph M. Sanzari Childrens Hospital. Broadening the scope of pediatric surgery specialties, K. Hovnanian Childrens Hospital added pediatric urologists and pediatric plastic surgeons to their team of experts, and recruited additional physicians in pediatric surgery, pediatric neurosurgery, and pediatric orthopedics. These specialists are now offering 24/7 on-call coverage at K. Hovnanian Childrens Hospital, where a Surgical Physician Assistant Program provides enhanced care to patients undergoing surgery. These programs and physicians are enriching the expansive breadth of pediatric services provided through the network; delivering highly sought after specialty care to pediatric patients and their families. Hackensack Meridian Health is deeply committed to supporting health and wellness initiatives for the communities we serve, and children continue to be a key focus. Through education programs, community outreach events, and nutrition and fitness services, Hackensack Meridian Health collaborates with local organizations, schools, and community partners to combat childhood obesity, and inspire our youth to lead vibrant, healthy lives. Educating the Next Generation of Physicians =========================================== A strategic mission of Hackensack Meridian Health is to develop into a major academic center. Hackensack Meridian Health and Seton Hall University are in a unique position to partner in the development of an innovative New Jersey-based health education program to address the upcoming physician shortage. This is why they are moving forward to establish a new, four-year school of medicine. Both organizations believe their academic reputations, combined with clinical expertise, will create an opportunity to form New Jerseys first private school of medicine that will no doubt become one of the premier schools in the nation. The proposed school of medicine would be dedicated to achieving the following goals: educational excellence; cutting-edge research; highest-quality clinical care; meaningful integration; and the advancement of New Jersey medical and health sciences educational imperatives. Seton Hall University will bring its College of Nursing and its School of Health and Medical Sciences, which includes its Physician Assistant Program, Athletic Training, Occupational Therapy, Physical Therapy, Speech and Language Pathology, Masters in Health Administration and a Ph.D. in Health Sciences (research) to this new health sciences campus. This will create an opportunity to integrate the education and training of these multiple disciplines into a single classroom setting a truly innovative approach that will mirror how health care will be delivered in the future.
CORE FORM, PART III - CONTINUED The new school of medicine will bring together the best of both worlds: a state-of-the-art, one-of-a-kind campus, coupled with Hackensack Meridian Healths flourishing research partnership with Georgetown University. This will create a world-class network of academic, research and clinical expertise to educate the next generation of physicians. Students will be trained within New Jersey on Hackensack Meridian Health campuses, and retention programs will be developed to embed the physicians within the Network. Seton Hall Hackensack Meridian School of Medicine will be governed by a Board of Trustees with equal representation by Seton Hall University and Hackensack Meridian Health in terms of the voting power and number and types of individuals appointed by, and with representation meeting the requirements of the Liaison Committee on Medical Education. The medical school Board of Trustees will be responsible for overseeing and making recommendations regarding the strategies, policies and finances of the medical school. Partner Companies ================= 2016 proved to be an outstanding year for the partner companies as they continue to achieve positive financial results while continuing to deliver the highest level of care. The partner companies have led the way in Hackensack Meridian Healths expansion of programs and services outside of the traditional Monmouth and Ocean counties markets with many services already operating in other counties. As an example, Hackensack Meridian Health At Home, the largest provider of home health care services in Monmouth and Ocean counties, has partnered with JFK Health to expand its network of in-home health services to patients and families in Middlesex, Somerset and Union Counties. Since our merger in 2016, JFK At Home has experienced increased referral volume by obtaining cases from Raritan Bay Medical Center. Hackensack Meridian Health also has a service partnership with Alert Ambulance Service. With more than 100 vehicles, Alert is the largest regional ambulance company in New Jersey, providing approximately 78,000 transports annually. The company has experienced growth in 2016, much of it from assuming responsibility for ambulance services at Raritan Bay Medical Center. In addition, Alert has also recently started providing ambulance services at Palisades Medical Center. The merger with Raritan Bay Medical Center and Hackensack University Health Network provides these partners with a broad continuum of services and the opportunity for exponential growth in new markets. Hackensack Meridian Health At Home has developed and launched the Safe at Home Memory Care program, the first of its kind in the region, to assist patient and caregivers from the earliest stages of diagnosis and evaluation, through the many phases of memory care planning and management. The program has trained 68 nurses, 101 CHHAs, and 22 therapists in Dementia Capable Care. This training focuses on what people with Alzheimers and related dementias (ADRD) can still do instead of what they cant do. This training empowers our organization to provide unsurpassed person-centered care that helps persons with ADRD to thrive, not just survive. The Villas at Manalapan, an upscale vibrant boutique assisted living community set on seven acres off of Gordons Corner Road, is expected to launch in March 2018. The Villas at Manalapan is uniquely focused on our Revite Wellness Health Program focused on prevention, wellness and the reversal of chronic disease. Personalized health and wellness plans, including individualized nutrition and fitness programs, a full spa and dedicated health program will be developed for all residents. The 100,000-square-foot facility will house 96 residences including independent living apartments and assisted living and memory care residences, and will offer luxurious amenities beyond what most senior living communities offer to ensure an extraordinary experience. The facility will include gourmet fine dining with an open view into the kitchen serving fresh and seasonal meals, a full lounge and cocktail bar, fitness center, beauty salon, full spa, a cinema theater and a great room to enjoy musical concerts, performing arts and lectures. Outdoors, residents can enjoy beautiful gardens, walking paths, patios and an outdoor fire pit. It is our belief that if you feel well, you can live well, and we are excited to be developing these new residences for the community. Hackensack Meridian Health Nursing & Rehab facilities at Brick, Shrewsbury, Wall, Ocean Grove and Bayshore in Holmdel, along with The Harborage in North Bergen, received the highest overall rating of five stars in U.S. News & World Reports eighth annual Best Nursing Homes. These five-star ratings underscore the high-quality care we consistently deliver to our patients across New Jersey. Hackensack Meridian Health formally launched its Integrative Health & Medicine program in Monmouth, Ocean and Middlesex counties last July with the opening of the Center for Integrative Health & Medicine at Hackensack Meridian Health Village at Jackson. The new center and program empowers patients to reach the highest expression of themselves, engaging their mind, body and spirit to achieve optimal health and improved quality of life. The Hackensack Meridian Center for Integrative Health & Medicine couples traditional clinical medicine with healing therapies, treating the whole patient and not just their illness or disease. It is the only integrative health program to be built upon five Pillars of Wellness Sleep, Activity, Purpose, Nutrition and Resilience. At the Center for Integrative Health & Medicine, patients can address their health concerns and goals, and receive a customized health and wellness plan developed by the centers integrative specialists. The centers integrated health physician, health coaches, nutritionist, health psychologist, acupuncturist and other professionals work with the patient and their specialty physicians to help each patient achieve their health and wellness goals. The opening of the center demonstrates our dedication to a comprehensive, patient-centered approach to health care. Providing the community with these unique, innovative services will empower them to take control of their health. The creation of this program in Monmouth, Ocean and Middlesex counties was made possible thanks to philanthropic members of the community. Their thoughtful contributions will greatly impact the way health care is delivered in New Jersey. Population Health ================= Beckers Hospital Review featured Hackensack Meridian Healths Accountable Care Organizations (ACOs) on its 100 ACOs to Know for 2016 list. Beckers featured Hackensack Meridian Health on its national list for its strong portfolio of ACOs, including the commercial clinically integrated network, Hackensack Meridian Health Partners, as well as the networks two Medicare ACOs, HackensackAlliance ACO and Meridian ACO. Combined these ACOs boast some of the strongest quality scores in the nation and are responsible for millions of dollars in health care savings. Hackensack Meridian Health Partners entered new territory in 2016, expanding from Monmouth and Ocean counties to Bergen, Essex, Hudson, Middlesex and Passaic counties. Physician membership experienced its biggest growth to date, hitting more than 2,000 members and growing by nearly 60 percent in just three months. This exponential growth connects more Hackensack Meridian Health primary care and specialists, enabling the delivery of coordinated and efficient care to patients across seven counties. Across the network, leaders, nurses and physicians are collaborating to improve patient care and redesign Hackensack Meridian Healths care coordination. This care coordination project demonstrates Hackensack Meridian Healths commitment to enhancing patient care throughout the care continuum not just when theyre in the hospital. As a first step to help create smoother care transitions and improve coordination, Hackensack Meridian Health care teams are implementing a new technology, PatientPing. PatientPing gives providers real-time notifications whenever their patients receive care at a wide range of facilities in New Jersey and beyond. Meanwhile, PatientPing gives care instructions to the facility where patients are receiving care, including contact information for the patients care team as well as the patients visit histories. This real-time sharing of information enables providers to apply timely interventions, avoid unnecessary readmissions, and ensure safer care transitions for their patients. Hackensack Meridian Health is the first health care provider to use this technology in New Jersey. This care coordination endeavor will help streamline and align care patient care.
CORE FORM, PART III - CONTINUED Hackensack Meridian Healths two Medicare Accountable Care Organizations (ACO) hit new milestones this year. HackensackAlliance ACO experienced its most successful year ever, generating shared savings for the third year in a row. HackensackAlliance ACO ranked seventh in the nation for total savings, saving more than $33 million for the 2015 Performance Year. In addition, it improved its quality score to a 95.7 percent overall Quality Score. Meanwhile, Meridian ACOs overall Quality Score for the 2015 Performance Year was 92.71 percent, making it the third consecutive year in which the Meridian ACO realized year-over-year improvements in quality. Hackensack Meridian Health is participating in two Medicare initiatives aimed at improving patient care, outcomes and satisfaction. Nearly all of Hackensack Meridian Healths hospitals are participating in Medicares Comprehensive Care for Joint Replacement (CJR) model. The major orthopedic program is designed to improve the care and outcomes of Medicare patients undergoing hip and knee replacements. In less than a year, the team comprised of clinical, administrative and executive team members worked tirelessly to increase coordination and implement new tools to support our care of patients undergoing hip and knee replacements. The CJR model started on April 1, 2016, and will last for five years. Community Benefits ================== Our mission to provide the full spectrum of life-enhancing care and services to create and sustain healthy, vibrant communities is at the heart of our charitable roots. Hackensack Meridian Health is leading the way for health cares expansion beyond hospital beds and into communities, where patients and experts come together to form the ultimate health networking hub. Its all part of our mission to redefine traditional health care through technology and community, further enhancing engagement between hospitals, physicians and most importantly, patients. As a non-profit health care provider, Hackensack Meridian Health is the regional leader in providing innovative and accessible health care programs and services to individuals, families and communities throughout the state. Everyone deserves access to quality care regardless of their ability to pay. In 2016, Hackensack Meridian Health provided $158 million in charity care and other uncompensated care, serving as a health care safety net for our communitys most vulnerable populations. In addition, Hackensack Meridian Health dedicated $31.8 million in subsidizing vital health services, such as outpatient dialysis, behavioral health services and family health clinics. In total, Hackensack Meridian Health devoted more than $255 million in community benefits in 2016. Hackensack Meridian Health plays a lead role in working with many different organizations throughout New Jersey to identify and address the health issues that impact our communities the most. This collaborative effort is referred to as a Community Health Needs Assessment. With the Community Health Needs Assessment as our guide, Hackensack Meridian Health prepares its annual Community Benefits Plan, which is part of the overall strategic plan, and aligns activities and resources toward those priority health needs. Each hospital has prepared an implementation strategy that contains the specific programs and resources that will be deployed against each health priority including community partners with whom we collaborate on interventions. Findings from the assessments highlighted several health concerns for our community, including access to health care services, cancer, diabetes, heart disease and stroke, injury and violence, mental health, nutrition, physical activity and weight, respiratory diseases and substance abuse. Preventing Disease Through Early Detection ------------------------------------------ Early detection is the best prevention. Our preventive health screenings include AngioScreen stroke risk assessments, blood pressure, cholesterol, glucose, BMI, memory, hearing, diabetic retinopathy, bone density, breast, colorectal cancer, skin cancer, and more. Participants receive individualized testing from medical professionals and are counseled by nurse educators who provide information about their results and referrals for follow-up care when necessary. Cancer Prevention and Awareness Hackensack Meridian Health partners with many different community organizations who share the mission of reducing New Jerseys cancer burden and improving health outcomes. Skin Cancer Ocean Monmouth Health Alliance (OMHA) is a regional chronic disease prevention coalition funded by the Office of Cancer Control and Prevention of NJ Department of Health. Hackensack Meridian Health partners with OMHA on their annual Choose Your Cover skin cancer screening at beaches, parks and other locations across the state. In 2016, there were 272 individuals who were screened at three locations. Of those, 11 basal cell carcinoma cases, three squamous cell carcinoma cases and one melanoma case were detected. The John Theurer Cancer Center, the Promise Foundation of Ridgewood, NJ, and Hackensack Meridian Health collaborate to provide an educational initiative to teach youth sun-safe behaviors. In 2016, over 5,000 middle school students participated in this program and learned how to be sun safe. Breast Cancer Hackensack Meridian Healths Every Woman Counts program, funded by a grant from Susan G. Komen, increased annual mammography screenings for low-income, uninsured, and underinsured women in Monmouth and Ocean counties by collaborating with local non-profit organizations that are successful in engaging hard-to-reach communities such as: elderly, Hispanic, black, LGBT, undocumented immigrants, and the homeless. Through this collaborative program, culturally and linguistically appropriate breast health education was provided to over 1,400 women, 75 percent of whom demonstrated an increase in knowledge about breast health. A total of 400 mammogram screening vouchers were distributed and 150 diagnostic screenings were provided for women who require additional testing due to screening abnormalities. Transportation assistance for mammogram appointments and navigation throughout the continuum of care was also provided. Colon Cancer Colon cancer is one of the most preventable and, if detected early, the most treatable forms of cancer. In 2016, there were 419 community members who attended a seminar and 201 take-home colorectal kits were distributed. Of those, 89 kits were returned and nine cases tested positive for cancer. Each of these community members were paired up with a Hackensack Meridian Health cancer navigator to assist in obtaining the necessary diagnostic testing and treatment. Managing Chronic Disease ------------------------ According to the CDC, chronic disease accounts for seven in 10 deaths each year and accounts for 86 percent of the nations spending on health care. Half of all American adults have at least one chronic condition and almost one in three have multiple chronic conditions. Hackensack Meridian Health focuses much of its community health education on managing and preventing chronic disease. Take Control of Your Health Take Control of Your Health is an evidence-based chronic disease self-management program. Complementing a physicians plan of care, this educational program offers participants with the tools, resources and support to live a healthier life. Studies have shown that participants who complete the training report improved health status and quality of life, greater energy, fewer social limitations as well as reduced hospitalizations and fewer emergency room visits. Take Control of Your Health is also offered for diabetes patients and caregivers, and focuses on the unique needs of those individuals. Community Outreach for Asthma Care and Health (COACH) COACH at K. Hovnanian Childrens Hospital at Jersey Shore University Medical Center is a program designed to improve the quality of life for children with asthma and their families. Its all about teaching asthma self-management and reducing exposure to asthma triggers. Hospital-based services provided by COACH include patient/family centered asthma education for all children with an admitting diagnosis of asthma, post-hospital telephone follow up, and home visits for at-risk populations to reinforce asthma self-management and home assessment for triggers. Trigger remediation equipment such as vacuums and dust mite covers are provided for those in most need.
CORE FORM, PART III - CONTINUED Community-based services include a pediatric asthma clinic at Booker Family Health Center; a monthly asthma program at the K. Hovnanian Wellness Center, Midtown School in Neptune where children in the district receive a complete asthma assessment by a pediatric pulmonologist, which includes pulmonary function tests with education and follow up by a member of the COACH Team; an in-school Asthma Education program for middle school children with asthma where students are taught self-management skills and are encouraged to share their experiences living with asthma in an informal setting; educational lectures for school nurses in the surrounding communities; and parent programs in schools and at work as well as participation in community health fairs and other events. Health Promotion Providing Opportunities to Learn ---------------------------------------------------- Its never been easier to connect with Hackensack Meridian Health. Opportunities to learn can be found at our hospitals, conference centers and Fitness & Wellness locations, as well as right in the community the local library, senior center, school, employer, and now even at the shopping mall. Hackensack Meridian Healths Community Outreach team has received special training in health literacy as well as cultural competence to ensure the best possible experience for our diverse community. Learning Garden at Raritan Bay Medical Center Hundreds of organic produce, including purple potatoes, eggplants, spicy lettuce, collard greens, peppers, cherry tomatoes and more, were distributed to patients, volunteer gardeners, team members and others as part of Raritan Bay Medical Centers The Learning Garden. The container garden education program began in the summer of 2015 with a goal of providing hands-on education for children and adults about the basics of gardening and the importance of healthy produce and proper nutrition. Raritan Bays Integrative Health Services spearheaded the creation of the garden through a donation from the medical centers medical staff, with help from numerous team members and community volunteers and under the guidance of a Master Gardener. The Learning Garden serves many purposes. It provides an opportunity for everyone to not only learn the basics of gardening and nutrition, but to also get some exercise, socialize and empower community members to create their own gardens at home. The community and team members participated in many free gardening education sessions and lunch and learn events over the summer, learning how to garden and about the nutritional and medicinal value of fresh produce. Centering Pregnancy Jane H. Booker Family Health Center is proud to be a Certified Centering Pregnancy site. Centering Pregnancy is a unique model of prenatal care that empowers women to choose health promoting behaviors. This multifaceted model of group care, provided in both Spanish and English, integrates three major components of care: health assessment, education and support. Pregnant women meet together, learn self-care and parenting skills, participate in a facilitated discussion about prenatal care, birth and infant care, and develop a support network with other group members. The group meets for eight to 10 sessions throughout the prenatal period and is facilitated by both certified staff members trained in Centering Pregnancy, and physician providers or a nurse midwife. Participants boast a 99 percent satisfaction rate of their experience in the Centering Pregnancy program. This program was also part of a CMS demonstration project called Strong Start. The Pawsitive Action Team Over 10,000 children were educated on how to eat right, stay fit, be safe and act responsibly. Our childrens health education programs include classroom-based presentations from The Pawsitive Action Team, including Doctor Bernard, Hopscotch and Picatso, mascots for the K. Hovnanian Childrens Hospital at Jersey Shore University Medical Center. Community of LifeSavers Cardiac arrest is more common than you might think. It can happen to anyone at any time, including our youth. In fact, the American Heart Association (AHA) estimates that about 100 student athletes will die from sudden cardiac arrest each year. While the incidence remains low, the impact on the family and surrounding community is tremendous. Thats why Hackensack Meridian Health created a landmark, multi-year initiative to Create a Community of LifeSavers. When ordinary people are equipped with the skills to perform CPR, survival rate can double or even triple. Nearly 5,000 students are trained each year in CPR and AED use. This program is offered at no cost to the school or student. In addition to helping schools comply with Janets Law, the program is designed to build a sustainable CPR training infrastructure by training staff from each school district as a CPR instructor, enabling them to train additional teachers, administrators and students in their district. Providing broad CPR training to the community has been part of Hackensack Meridian Healths educational offerings for years, including health care workers, fire fighters, police, teachers, EMTs, and community members. Our community now has an army of rescuers who are ready, willing and able to put their skills in action in the event of an emergency. Hope Unseen Over 200 community members, many blind or visually impaired, participated in an inspiring and educational program made possible through funding from the Mitchell-Vassar Bright Future Legacy. Having a purpose is what makes life meaningful, but it is a choice.
