(1) Child and Family Support Assistance 109   15,903 FMV Funds assist families with date books, clothing, DVM fees, auto repairs, groceries/household items, medicine co pay, eye glasses, art supplies, court fees, families activities, respite reimbursements including camp, child care, after school programs, riding lessons, martial arts classes and hotel for VFAFA.
(2) Youth Development Assistance 56   640 FMV Funds assist youth with activities, bus tickets, co-pays, transportation, clothing and meals/groceries.
(3) Direct Assistance to Military and Veterans Services and their Families 48   17,286 FMV Funds assist veterans and current service members and their families who are in pre, current or post-deployment with critical and emergency needs including auto, food, medical/dental, rent, utilities and household items.

Part I, Line 2: Easter Seals assistance to individuals is intended to be a "hand up not a "hand out and is used as part of a comprehensive plan for achieving self-sufficiency and well-being. Assistance provided to individuals is monitored by the Organization by the following methods: 1. The individual is required to provide a receipt or some other sort of appropriate documentation which is then reviewed and approved by the Organization; and 2. Amounts are paid directly to the service provider and/or vendor to ensure funds are expended appropriately.
