FORM 990, PART III, LINE 4A Program Service Accomplishments Sinai Health System is the parent organization and provides administrative management support to its affiliated organizations to enable them to improve the health and communities it serves. Located on the west side of the city of Chicago and serving a population base of 1.5 million people in its service area ZIP codes, Sinai Health System (Sinai) consists of six member organizations. They are Mount Sinai Hospital, Holy Cross Hospital, Schwab Rehabilitation Hospital, Sinai Community Institute, Sinai Medical Group, and Sinai Urban Health Institute. Sinai has a number of clinics that are located on the west and south sides of Chicago. There is also one facility, Sinai's Touhy Clinic on the north side of Chicago, which sees refugees from many nations (for example Sudan, Iraq and Burma) as well as resident Orthodox Jewish and Russian populations. With the exception of the Touhy Clinic, Sinai Health System serves primarily African-American and Latino patients. Sinai's programs and community outreach are based on scientific epidemiological assessments of local neighborhoods done by Sinai Urban Health Institute (SUHI). In addition to the work of epidemiologists, SUHI community health educators work with neighborhood groups and go door to door for further identification of individuals with chronic disease and for intervention in disease processes. SUHI also conducts health disparities research, develops innovative community health interventions, delivers community health worker training and consultation, and provides a broad scope of evaluation services. Part III, Line 4a and 4b describe the top program services that Sinai conducts with SUHI.
FORM 990, PART III, LINE 4B PROGRAM SERVICE ACCOMPLISHMENTS Breast cancer is the second leading cause of cancer death among US women. Breast cancer mortality can be reduced through use of mammography and through treatment, both of which can be increased through targeted outreach. Determining which groups experience high rates of breast cancer prevalence and mortality can help guide outreach and intervention efforts. Research shows race/ethnic differences in breast cancer incidence and mortality. Hispanic women in the US generally display lower rates of breast cancer incidence and mortality than their non-Hispanic counterparts. However, we know that examining data at the Hispanic aggregate level masks important differences among subgroups. For example, studies of stroke and diabetes mortality among US Puerto Ricans and Mexicans reveal much higher rates among Puerto Ricans compared to their Mexican and Hispanic counterparts. Thus, it is critical to examine Hispanic subgroups individually. This project seeks to improve our understanding of the breast cancer prevalence and mortality rates among Hispanic subgroups in the US. Specifically, the primary aim is to calculate the breast cancer prevalence and mortality rates of Mexican, Puerto Rican, Cuban, Central American, South American, and Latin American women in the US. We will determine whether any of the between-group differences are statistically significant to highlight where targeted outreach efforts may be needed for groups with significantly higher mortality rates.
FORM 990, PART III, Line 4C Program Service Accomplishments Sinai Health Systems Mount Sinai Hospital and Holy Cross Hospital are located in community areas with high incidences of HIV. Routine HIV screening efforts in these two hospitals identify about 200 HIV positive individuals annually. In order to provide treatment and other needed social services to these individuals, the Mount Sinai Hospital Infectious Disease Center (MSHIDC) receives grant funding for outpatient ambulatory health services, as well as case management services. The main goals of these services are to: 1.Provide patients with medical care to manage their HIV 2.Provide patients with case management services to help them address barriers to care 3.Achieve virologic suppression through medication adherence Sinai practices routine screening to identify newly diagnosed and known HIV positive individuals. We currently offer case management services to all (both new and known) HIV positive patients to help ensure medication adherence. Our clinic population is comprised of patients who come from the community and is 78% Black, 21% Latino, 25% MSM, and 48% cisgender women. In 2017, 18% of the patients seen in the MSHIDC were Black MSM.
FORM 990, PART VI, LINE 1A Explanation of Delegated Board Authority to Committee The Executive Committee consists of the Board Chair, Vice Chair(s), Secretary, Treasurer and President. The Executive Committee has the power to transact all regular business of the Sinai Health System during the period between meetings of the Board, subject to any prior limitation imposed by the Board or by law. When action is taken by the Executive Committee, it will be reported to the Board at the next meeting of the Board.
FORM 990, PART VI, LINE 7A Description of classes of persons and the nature of their rights Board members are nominated by the Board Recruitment, Education, and Nominating Committee of Sinai Health System, and elected by the members from among those persons approved for appointment by Sinai Health System.
FORM 990, PART VI, LINE 11B Describe the process used by management &/or govening body to review 990 The Form 990 information was initially provided by the finance staff with multi-disciplinary input from Public Affairs, Corporate Integrity and other appropriate staff of the organization. The Form 990 was then prepared by Ernst & Young, LLP and reviewed by senior finance staff and other members of senior leadership. Prior to the filing, the Form 990 was made available to the full board of directors.
FORM 990, PART VI, LINE 12C Describe the process to monitor transactions for conflicts of interest The Conflict of Interest Disclosure Form is completed and signed annually by all board members, employed physicians, management personnel as well as other employees who are in a position to influence purchasing decisions, affiliations or referrals, hiring decisions or contracts. The form is completed on appointment to the board of directors of Sinai Health System or any one of its entities, upon hire for any relevant job category or at the time of appointment to the medical staff. In addition to the annual filing of the form, an update is required to be filed any time there has been a change. Conflicts disclosed on the Conflict of Interest Disclosure Form are taken into consideration when making board committee assignments. In addition, individuals who have a conflict of interest must abstain from participating in decisions affecting the interested parties and make it clear why they are abstaining. If the potential for conflict of interest exists, employees and physicians are required to discuss the situation with management. Board members of the Sinai Health System or any of its entities are required to report potential conflicts to the Chief Integrity Officer who will review potential conflicts with the Sinai Health System Chief Executive Officer and Chairman of the Board of Directors.
FORM 990, PART VI, LINE 15A & 15B Compensation review & approval process - officers & key employees Sinai Health System has an Executive Compensation Committee to support the organizational performance of Sinai Health System and its related entities through the alignment of executive compensation with system strategies and programs and ensure compliance with applicable law. The voting members of the Committee are independent members of the Board of Directors. The Committee meets twice annually, or more frequently, as circumstances require. The Committee establishes detailed goals annually for the President and CEO and other executives, and reviews performance against these goals on an annual basis. The Committee annually engages an outside, independent compensation consultant to benchmark the salaries and benefits of the organizations assistant vice presidents and above, as well as a few directors. Compensation is based on detailed written performance appraisals and external market data. In executive session, the Committee reviews the performance of the President and CEO, each element of compensation, data of compensation programs in effect for CEOs of comparable organizations, and conducts an annual review of CEO performance against established goals. The Committee maintains written minutes which are maintained in Executive Administration.
