FORM 990, PART III, LINE 4A (CONT'D) DENTAL CARE: GOOD DENTAL CARE IS A PREREQUISITE TO GOOD HEALTH. IN FACT, STUDIES SHOW THAT CHILDREN WITH POOR ORAL HYGIENE ARE AT INCREASED RISK FOR HEART DISEASE, STROKE AND A NUMBER OF OTHER AILMENTS LATER IN LIFE. UNFORTUNATELY, MOST LOW-INCOME PEOPLE DO NOT RECEIVE ADEQUATE DENTAL CARE, AND PUBLIC INSURANCE PROGRAMS, SUCH AS MEDICAID AND MEDICARE, INCREASINGLY LIMIT THE DENTAL SERVICES THEY WILL COVER, PARTICULARLY FOR ADULTS. IN 2018, SHS PROVIDED COMPREHENSIVE DENTAL CARE (SCREENINGS, EXAMINATIONS, XRAYS, FILLINGS, EXTRACTIONS, SEALANTS, FLUORIDE VARNISHES, TREATMENT OF GUM DISEASE, AND DENTURES) TO CLOSE TO 8,500 ADULTS AND CHILDREN. WE OFFER SAME-DAY TREATMENT TO PATIENTS IN PAIN, PROVIDING AN ALTERNATIVE TO THE EXPENSIVE EMERGENCY DEPARTMENT AT A LOCAL HOSPITAL. WE WORK WITH OB/GYN PROVIDERS AND PEDIATRIC PROVIDERS TO SCHEDULE PRIORITY APPOINTMENTS FOR THESE DENTALLY VULNERABLE POPULATIONS. DURING THE PAST YEAR, THE DENTAL DEPARTMENT HAS INCREASED ITS MOBILE DENTAL SERVICES TO INCLUDE MULTIPLE HEAD START LOCATIONS TREATING PATIENTS AGE THREE TO FIVE; ATTENDING A CHARTER ELEMENTARY SCHOOL TO TREAT CHILDREN FROM KINDERGARTEN TO EIGHTH GRADE; AND TREATING PATIENTS AT OVER SEVEN HOMELESS SHELTERS THAT INCLUDES BOTH MEN AND WOMEN, INCLUDING A DOMESTIC VIOLENCE SHELTER FOR WOMEN. WE HAVE ALSO BEEN TREATING DENTAL PATIENTS AT ONE DAY EVENTS THROUGHOUT THE METROPOLITAN KANSAS CITY AREA FROM KANSAS TO MISSOURI. BEHAVIORAL HEALTH: ACCORDING TO A 2018 REPORT ENTITLED A VIEW OF CONSUMER HEALTH ACCESS IN KANSAS AND MISSOURI BY THE GREATER KANSAS CITY HEALTH CARE FOUNDATION, KANSAS HEALTH FOUNDATION, MISSOURI FOUNDATION FOR HELP, REACH HEALTHCARE FOUNDATION AND UNITED METHODIST HELP MINISTRY FUND, 30% OF ADULTS 19 TO 64 RESPONDING TO THE SURVEY HAD A MENTAL HEALTH DIAGNOSIS. IN 2018, SWOPE HEALTH SERVICES PROVIDED MENTAL HEALTH AND/OR SUBSTANCE USE DISORDER SERVICES TO NEARLY 6,300 PATIENTS RESULTING IN OVER 100,000 VISITS. MANY OF THESE CLIENTS HAVE VERY FEW RESOURCES AND HAVE A DESPERATE NEED FOR THE BEHAVIORAL HEALTH CARE THAT SHS PROVIDES, WHICH CONSISTS OF VISITS FOR PSYCHIATRIC EVALUATION, MEDICATION MANAGEMENT, OUTPATIENT THERAPY, CASE MANAGEMENT, SUBSTANCE USE DISORDER TREATMENT INCLUDING OPIOID TREATMENT AND INTENSIVE RESIDENTTAL TREATMENT AS WELL AS SUPPORT FOR THOSE EXPERIENCING ACUTE CRISES. OPIOID TREATMENT IS EXPANDING AND BECOMING A SIGNIFICANT PART OF THE OVERALL SUBSTANCE USE DISORDER TREATMENT PROGRAM. DURING 2018, SWOPE HEATH SERVICES ALONG WITH OTHER COMMUNITY MENTAL HEALTH CENTERS AND A CONSORTIUM OF AREA HOSPITALS CONTINUED TO IMPROVE AND BUILD UPON THE HOSPITAL DIVERSION PROGRAM. THIS PROGRAM IS DESIGNED TO HELP THOSE SEEKING SERVICES VIA THE AREA HOSPITAL EMERGENCY ROOMS AND RECEIVE INTENSIVE CASE MANAGEMENT SERVICES AND TREATMENT ENGAGEMENT TO HELP BREAK THE CYCLE OF HOSPITALIZATIONS. THESE CLIENTS RECEIVE MUCH MORE COMPREHENSIVE TREATMENT BY THE COMMUNITY PROVIDERS, WHICH LEADS TO BETTER UTILIZATION OF THE RESOURCES AVAILABLE TO HELP THOSE IN CRISIS OR NEED. THE DATA SHOWS THAT THIS PROGRAM CONTINUES TO BE SUCCESSFUL. AS ONE OF THE FEW SAFETY NET AGENCIES IN THE GREATER KANSAS CITY AREA LOCATED IN THE URBAN CORE, SHS WORKS HARD TO ACHIEVE ITS MISSION TO HELP PROVIDE INTEGRATED MENTAL HEALTH, SUBSTANCE USE DISORDER AND PRIMARY CARE SERVICES TO A POPULATION AT RISK FOR UNEMPLOYMENT, VIOLENCE AND SHORTENED LIFE EXPECTANCY. IN 2018 THE ADDICTION TREATMENT CENTER IN THE CENTRAL KANSAS CITY AREA PROVIDED SUBSTANCE USE TRIAGE SERVICES TO HUNDREDS OF JACKSON COUNTY RESIDENTS. SWOPE HEALTH SERVICES ALONG WITH THE OTHER AREA PROVIDERS AND THE METROPOLITAN AREA POLICE DEPARTMENTS ARE PROVIDING A CRITICAL SERVICE TO HELP REDUCE THE INAPPROPRIATE USE OF HOSPITAL EMERGENCY ROOMS BY INDIVIDUALS THAT MAY BE EXPERIENCING A SUBSTANCE USE DISORDER PROBLEM THAT IS BETTER TREATED IN AN OUTPATIENT SETTING. CREATING A ONE-STOP, INTEGRATED HEALTH CARE SYSTEM: IN 2018 SHS DEDICATED A TEAM OF MEDICAL, BEHAVIORAL AND DENTAL CARE LEADERS TO DIRECT THE INTEGRATED CARE EFFORTS AT SWOPE HEALTH SERVICES. WHEN SEEKING SERVICES PATIENTS OFTEN MUST TRAVEL TO DIFFERENT SITES ON DIFFERENT DAYS TO GET ALL THE MEDICAL, DENTAL AND BEHAVIORAL HEALTH CARE THEY NEED. AS PART OF THIS EFFORT A MORE COORDINATED APPROACH IN PATIENT REFERRAL BETWEEN SERVICE AREAS IN SWOPE HEALTH SERVICES IS RESULTING IN GREATER CARE COORDINATION AND BETTER ACCESS TO CARE IN A MORE TIMELY MANNER. THIS EFFORT CONTINUES TO EXPAND. ONE OF THE NEW INITIATIVES WAS TO TAKE ALL THREE SERVICE AREAS AND CREATE A MEMORANDUM OF UNDERSTANDING TEMPLATE THAT CAN BE USED WITH AREA NOT-FOR-PROFIT AGENCIES IN ORDER TO HELP THEM BETTER IDENTIFY AND UTIILIZE THE INTEGRATED SERVICES OFFERED BY SWOPE HEALTH. HEALTHCARE HOME: SHS IS PARTICIPATING IN BOTH THE PRIMARY CARE AND BEHAVIORAL HEALTH HOME INITIATIVES SPONSORED BY THE STATE OF MISSOURI. THE PRIMARY CARE MEDICAL HOME HAS AN AVERAGE ENROLLMENT OF 660 CLIENTS AND THE BEHAVIORAL HEALTH ENROLLMENT AVERAGES 400 CLIENTS. THE FOCUS OF BOTH HEALTH HOMES IS GREATER UTILIZATION OF THE HEALTH HOME TEAM FOR TREATMENT OF CHRONIC DISEASES AND PREVENTION WITH LESS INAPPROPRIATE USAGE OF EMERGENCY CARE AND REDUCTION UNWARRANTED HOSPITALIZATIONS.

COMMENTS AND QUESTIONS OF THE REVIEWING INDIVIDUALS ARE ADDRESSED BY THOSE RESPONSIBLE FOR PREPARING THE FORM 990, AND APPROPRIATE MODIFICATIONS ARE MADE TO THE DRAFT BEFORE IT IS FILED.
FORM 990, PART VI, SECTION B, LINE 12C ALL DIRECTORS, OFFICERS, KEY EMPLOYEES, AND ASSOCIATES ARE REQUIRED TO COMPLETE ANNUALLY THE CONFLICT OF INTEREST DISCLOSURE FORM AND PROVIDE PERIODIC UPDATES TO THE FORM AS MAY BE NECESSARY. IF A CONFLICT IS IDENTIFIED VIA ANNUAL DISCLOSURE FORM, SUPERVISOR AND HUMAN RESOURCES WILL DISCUSS STEPS TO RESOLVE. DECISIONS WERE COMMUNICATED TO THE VP OF HUMAN RESOURCES FOR IMPLEMENTATION. THE DISCUSSIONS AND DECISIONS WERE CONTEMPORANEOUSLY DOCUMENTED.

FORM 990, PART XII, LINE 2C: THE ORGANIZATION'S GOVERNANCE STRUCTURE INCLUDES A FINANCE COMMITTEE, WHICH IS A SEPARATE STANDING COMMITTEE OF THE BOARD OF DIRECTORS.
