PART I, LINE 4A THE CHIEF STRATEGY OFFICER RECEIVED A SEVERANCE PAYMENT UPON DEPARTURE FROM THE ORGANIZATION IN THE AMOUNT OF $93,215.
PART I, LINE 7 THE CEO PARTICIPATES IN AN ANNUAL BONUS PROGRAM. ANY BONUS PAYMENTS ALLOWABLE UNDER THE PROGRAM ARE BASED ON CEO PERFORMANCE AND AT THE DISCRETION OF THE BOARD OF DIRECTORS. STAFF ARE ELIGIBLE TO PARTICIPATE IN AN ANNUAL BONUS PROGRAM. ANNUALLY, FUNDING IS ALLOCATED TO SUPPORT THE ANNUAL BONUS PROGRAM AND IS BASED ON A PRE-ESTABLISHED PERCENTAGE OF SALARY. ANY BONUS PAYMENTS ALLOWABLE UNDER THE PROGRAM ARE BASED ON AN EMPLOYEE'S PERFORMANCE AND AT THE DISCRETION OF THE ORGANIZATION.
