Form 990, Part VI, Section A, line 2 Lester Crown, A. Steven Crown, and Sara Crown Star have a family relationship.
Form 990, Part VI, Section A, line 6 Members of the corporation are divided into two classes per the Bylaws: the donor members and the publicly supported organization members which are the Jewish Federation of Metropolitan Chicago and the Jewish United Fund of Metropolitan Chicago.
Form 990, Part VI, Section A, line 7a The Board of Directors consists of nine persons: four appointed by the donor members, four appointed by the publicly supported organization members, and the ninth will be the person serving as the President of the Federation, or his or her designee. If the Federation ceases being a member of the Corporation, the successor will be elected by the publicly supported organization members.
Form 990, Part VI, Section B, line 11b A copy of Form 990 was provided to the Board Members for their review prior to filing with the IRS. The Board Members were able to make recommendations for any changes prior to filing. The Form 990 was reviewed and approved by the Compliance Committee.
Form 990, Part VI, Section B, line 12c A conflict of interest questionnaire is circulated to all Board Members (Directors) and key personnel on an annual basis. This process is managed by the Jewish Federation of Metropolitan Chicago (Federation). The completed questionnaires are reviewed by Federation management and any conflicts are disclosed to the Board of Directors. If a Director does have a conflict or business relationship with the Foundation, he or she is required to abstain from any votes related to to that matter.
Form 990, Part VI, Section C, line 19 The Foundation's governing documents, conflict of interest policy and financial statements are made available to all volunteer members of the Board of Directors. With regard to the general public, the organization provides these upon request for the same period of disclosure as set forth in IRC Section 6104(d).
