Form 990, Part VI, Section A, Line 6 The organization has members. The members of the Apostolic Christian Home of Eureka are the members of the four Apostolic Christian Churches that sponsor our organization. The churches are the Congerville IL, Eureka IL, Goodfield IL and Washington IL Apostolic Christian Churches.
Form 990, Part VI, Section A, Line 7a The members elect nine individuals from sponsoring churches to serve as our Board of Directors.
Form 990, Part VI, Section A, Line 7b The members make decisions regarding the continuation of our organization.
Form 990, Part VI, Section B, Line 11b Each Board member received a copy of the completed 990 form and related scheduled electronically. The Administrator reviewed the 990 form and related schedules with the Board during the monthly meeting held on April 18, 2022.
Form 990, Part VI, Section B, Line 12c The conflict of interest policy of the Apostolic Christian Home of Eureka requires that each officer and director complete a conflict of interest disclosure form annually and at any time during the year that a potential conflict arises. All potential conflicts must be disclosed to the President of the Board of Directors.
Form 990, Part VI, Section B, Line 15 Per the compensation policy of the Apostolic Christian Home of Eureka, the Board of Directors conducts a review of the Administrator's performance and salary on an annual basis. Comparable compensation data is obtained from a survey of comparable facilities. The Board of Directors vote the salary adjustments for the Administrator and document salary discussions and decisions within the Board minutes. In 2021, the Administrator's review was held on December 20th with follow up meeting on January 17, 2022. For all other positions within our organization, the Board of Directors and Administrator are responsible for setting a salary range based on current market conditions and surveying local comparable facilities. The Administrator is then responsible for ensuring that the individual salaries stay within the parameters of the salary ranges established by the Board of Directors for each position.
Form 990, Part VI, Section C, Line 19 The Apostolic Christian Home of Eureka's governing documents, conflict of interest policy and financial statements are available to the public by providing copies upon request or allowing inspection at the business office of our organization.
