Form 990, Part VI, Line 11b: Form 990 Review Process The process for reviewing the schedule 990 is to have the CPA present it to the finance committee of the board and then the finance committee presents the document in full at a bi-monthly board meeting. If members are not present they receive a copy of the 990 from the chairman of the finance committee.
Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of Conflicts The board of directors has a conflict policy that is distributed and reviewed during the first board meeting of the year and members are directed and reminded at that time to reveal all conflicts. Bi-monthly in person meetings take place with all board members in par to ensure compliance with this policy.
Form 990, Part VI, Line 15a: Compensation Review & Approval Process - CEO, Top Management The Ad hoc HR committee of the board with outside consultant members met to set salary for the CEO, Executive Director and others on staff in leadership positions.
