THE FOLLOWING EMPLOYEES RECEIVED TAXABLE HEALTH CLUB BENEFITS DURING FY 2021: EINAR BOHLIN: $600 ERIN ALLIGOOD: $300 GARTH DUBIN: $149
SCHEDULE J, PART II:
AS A GENERAL NOTE OF EXPLANATION FOR SCHEDULE J, PART 2 ON COMPENSATION, ARIN CONTRIBUTES 9% TO THE 401K RETIREMENT PLAN OF ANY EMPLOYEE WHO CONTRIBUTES 6% OF THEIR SALARY. THE MATCH IS 6% FOR THE FIRST 3% EMPLOYEE CONTRIBUTION AND THEN DOLLAR FOR DOLLAR UP TO 6% OF THE EMPLOYEE'S SALARY. IN ADDITION, ARIN CONTRIBUTES TO THE HEALTH INSURANCE COST OF EMPLOYEES. SOME EMPLOYEES HAVE OPTED OUT OF ONE OR BOTH OF THESE BENEFITS WHICH WOULD EXPLAIN ANY VARIATION IN THE AMOUNTS LISTED IN EITHER COLUMN C OR D, WITH THE RESULTING TOTAL SHOWING IN COLUMN E.
