COMMUNITY CARE IS THE EXCLUSIVE ORGANIZATION IN WISCONSIN TO OFFER EACH OF THE THREE LONG-TERM CARE PROGRAMS OF PROGRAM OF ALL-INCLUSIVE CARE FOR THE ELDERLY (PACE), FAMILY CARE AND PARTNERSHIP. OUR FAMILY CARE MEDICAID PROGRAM FOR FRAIL ELDERS AND ADULTS WITH INTELLECTUAL, DEVELOPMENTAL OR PHYSICAL DISABILITIES OFFERS THE FLEXIBILITY, EXPERTISE AND SERVICE OPTIONS TO HELP MEMBERS LIVE AS INDEPENDENTLY AS POSSIBLE FOR AS LONG AS POSSIBLE AT HOME OR IN THE COMMUNITY. THE FAMILY CARE PROGRAM OFFERS THE ADVANTAGES OF HAVING A TEAM OF PROFESSIONALS GUIDE MEMBERS. OUR MEMBERS WORK WITH THEIR TEAM TO DEVELOP A COST-EFFECTIVE PLAN THAT INCLUDES NATURAL SUPPORTS AND PAID SERVICES FROM OUR EXTENSIVE NETWORK OF SERVICE PROVIDERS. WE ARE ABLE TO TAILOR SERVICES TO OUR MEMBERS INDIVIDUAL NEEDS, CIRCUMSTANCES AND PREFERENCES. THE COMMUNITY CARE PACE PROGRAM IS THE ONLY PACE PLAN IN THE STATE OF WISCONSIN. PACE OFFERS A BENEFIT PACKAGE THAT COMBINES MEDICAL CARE, LONG-TERM CARE AND PRESCRIPTION DRUGS. PACE HELPS FRAIL AND DISABLED INDIVIDUALS AGE 55 AND OLDER LIVE AS INDEPENDENTLY AS POSSIBLE FOR AS LONG AS POSSIBLE WITHIN THE COMMUNITY. SHOULD THE ORGANIZATION BECOME AWARE OF ANY UNREPORTED POTENTIAL CONFLICTS, THE CONCERNS ARE RAISED WITH THE INDIVIDUAL TO DETERMINE THE EXISTENCE OF ANY SUCH ISSUE. THE DISCLOSURES ARE REVIEWED BY THE COMPLIANCE OFFICER AND THE BOARD CHAIRPERSON TO DETERMINE IF ANY MITIGATION IS REQUIRED. THE FOLLOWING DESCRIBES HOW COMMUNITY CARE INC. DETERMINES ITS SALARY AND WAGES. COMMUNITY CARE, INC. ENGAGED THE SERVICES OF A CONSULTING FIRM TO PROVIDE SALARY RANGE SUGGESTIONS FOR TOP PERSONNEL AND FOR MANAGERS IN THE HUMAN RESOURCES DEPARTMENT. IN 2021, THE CONSULTING FIRM ANALYZED ALL POSITIONS WITHIN A DEFINED SCOPE AND CLOSELY REVIEWED THE ASSOCIATED JOB DESCRIPTIONS. INITIAL COMPENSATION IS BASED ON TOTAL YEARS OF RELEVANT EXPERIENCE; ANNUAL MERIT INCREASES ARE BASED ON THE INDIVIDUAL'S PERFORMANCE EVALUATION, THEIR COMPA RATIO, AND AN ORGANIZATIONAL MERIT BUDGET. EACH PERFORMANCE EVALUATION IS REVIEWED AND APPROVED BY THE EXECUTIVE MANAGING THE ADMINISTRATOR OR DIRECTOR COMPLETING THE EVALUATION. THE HUMAN RESOURCE OFFICER THEN REVIEWS THE PERFORMANCE EVALUATIONS. SALARIES OF MANAGERS IN THE HUMAN RESOURCE DEPARTMENT ARE INCLUDED IN A MARKET STUDY TO AVOID ANY PERCEPTION OF A POTENTIAL CONFLICT OF INTEREST, AS THE DEPARTMENT SETS THE RANGES FOR ALL OTHER POSITIONS IN THE ORGANIZATION. |
